Abstract: Systems and methods for volumetric modulated arc therapy (VMAT) treatment planning include a processor determining, using a current solution of a non-convex VMAT optimization problem, a search region defining a corresponding spatial movement range for each leaf of a plurality of leaves of a MLC. The current solution can include first positions of the plurality of leaves of the MLC. The processor can merge, for each spatial movement range of a corresponding leaf, beamlets associated with the spatial movement range, and transform the nonconvex VMAT optimization problem into a convex VMAT optimization based on the merging of b camlets associated with each spatial movement range. The processor can solve the convex VMAT optimization problem to determine at least second positions of the plurality of leaves of the MLC.

Abstract: An example particle therapy system includes a particle accelerator to output a particle beam having a spot size; a scanning system for the particle accelerator to scan the particle beam in two dimensions across at least part of a treatment area of an irradiation target; and an adaptive aperture between the scanning system and the irradiation target. The adaptive aperture includes structures that are movable relative to the irradiation target to approximate a shape to trim part of the treatment area. The part of the treatment area has a size that is based on an area of the spot size.

Abstract: A volumetric modulated arc therapy (VMAT) treatment plan may be optimized by obtaining a VMAT treatment plan and calculating a radiation dose matrix corresponding to each a plurality of beamlets, wherein each beamlet represents a change in field when an MLC leaf is moved a predetermined unit distance. The method includes defining an enhanced objective function (EOF) for achieving one or more clinical objectives and minimizing the EOF for proposed leaf positions iterating through each leaf of at least a subset of the leaves of the VMAT treatment plan (wherein the proposed leaf positions move each leaf into the field or out of the field by the predetermined unit distance and correspond to the addition or subtraction of the corresponding radiation dose matrix). The set of leaf positions of the VMAT treatment plan is updated according to the proposed leaf positions of the minimized EOF.

Abstract: Methods and apparatus are provided for planning and delivering radiation treatments by modalities which involve moving a radiation source along a trajectory relative to a subject while delivering radiation to the subject. In some embodiments the radiation source is moved continuously along the trajectory while in some embodiments the radiation source is moved intermittently. Some embodiments involve the optimization of the radiation delivery plan to meet various optimization goals while meeting a number of constraints. For each of a number of control points along a trajectory, a radiation delivery plan may comprise: a set of motion axes parameters, a set of beam shape parameters and a beam intensity.

Abstract: A method for dose-gradient based optimization of an intensity modulated radiation therapy plan. First, an optimizer (6) performs a first optimization (40) of the plan to generate dose distributions corresponding to the plan. Next, the optimizer (6) generates a beam specific dose gradient map (42) for each beam of the plan. Then, new dose gradients are specified (44) for the plan. Last, the optimizer (6) performs a final optimization (46) using the new dose gradients. The final optimization is given the new dose gradients as soft constraints into an objective function. The optimizer (3) applies a limiting factor to the objective function such that a first dose gradient is limited by the optimizer only if the first dose gradient exceeds the new dose gradient for a specific beamlet.

Abstract: Methods for planning delivery of radiation dose to a target region within a subject comprise: iteratively optimizing a simulated dose distribution relative to a set of one or more optimization goals comprising a desired dose distribution in the subject over a first plurality of control points located on a trajectory, the trajectory comprising relative movement between a radiation source and the subject; reaching one or more initial termination conditions, and after reaching the one or more initial termination conditions: specifying a second plurality of control points along the trajectory and comprising a larger number control points than the first plurality of control points; and iteratively optimizing a simulated dose distribution relative to the set of one or more optimization goals over the second plurality of control points to thereby determine a radiation delivery plan.

Abstract: Systems for delivering radiation dose to a target area within a subject comprise: a radiation source for outputting a radiation beam; a support for supporting the subject; a movement mechanism for moving the radiation source relative to the subject along a trajectory; and a controller configured to cause, according to a radiation delivery plan: the radiation source to deliver the radiation beam to the subject while causing the movement mechanism to effect relative movement between the radiation source and the subject along the trajectory; and the radiation source to vary an intensity of the radiation beam by varying a radiation output rate of the radiation source according to the radiation delivery plan over at least a portion of the trajectory while causing the movement mechanism to effect relative movement between the radiation source and the subject, to thereby deliver dose to the subject according to the radiation delivery plan.
