摘要: An endoscope can include an elongated endoscope body. An optical fiber can extend along the endoscope body. The optical fiber can direct therapeutic light and illumination light longitudinally along the optical fiber to a distal portion of the endoscope body. The therapeutic light and the illumination light can have different wavelengths. A wavelength-sensitive light separator, disposed at a distal portion of the optical fiber, can direct the illumination light to exit the optical fiber laterally through a lateral side of the optical fiber at the distal portion of the optical fiber and permit the therapeutic light to exit the optical fiber longitudinally through a distal end of the optical fiber. Examples of suitable wavelength-sensitive light separators can include one or more fiber Bragg gratings that can be obliquely angled, or a diffraction grating disposed on a lateral edge of a length of coreless fiber.

摘要: Systems, devices, and methods for identifying different structure types with distinct composition in vivo and adjusting surgical laser output accordingly in a medical procedure are disclosed. An exemplary laser treatment system comprises a laser system configured to generate a laser beam for delivery to a target in a body, and a controller circuit configured to receive a signal reflected from the target in response to electromagnetic radiation produced by a light source, and generate one or more spectroscopic properties from the reflected signal. The controller circuit can identify the target as one of a plurality of structure types, such as tissue types or calculi types with respective compositions, using the one or more spectroscopic properties. The laser system can be controlled to operate in an operating mode based on the target identification.

An exemplary device comprises a proximal end, and distal end, and a transition section between the proximal and distal ends. The proximal end includes a first connector to be connected to a light source and a second connector configured to be connected to a spectrometer. The distal end includes a shaft including at least two first optical fibers to transmit light and at least one second optical fiber to transmit a spectroscopic signal. The transition section can couple the first connector to the at least two first optical fibers, and to couple the second connector to the at least one second optical fiber.

摘要: Systems, devices, and methods for identifying a target during an endoscopic procedure are disclosed. An endoscopic target identification system includes an endoscope having an endoscopic illumination source, an optical fiber insertable through a working channel of the endoscope and coupled to a non-endoscopic illumination source different from the endoscopic illumination source, and a controller. The controller can send a control signal to the non-endoscopic illumination source to emit a diagnostic beam through the optical fiber when the at least one endoscopic illumination source changes from a first mode to a second mode. The second mode has a lower amount of illumination than the first mode. The controller can determine a composition of a target based on the diagnostic beam incident on the target and light from the diagnostic beam being reflected from the target.

摘要: A single-use or reusable cap or an objective head to a scope can be used for integrating a laser fiber within the objective head but outside a working channel of the scope. The endoscope maintains an attached or embedded permanent laser fiber to ablate particulates that are suctioned into the endoscope assembly via a suction conduit. With such integration, there is no need to manipulate the laser fiber into a working channel; rather, a practitioner, e.g., physician, can move the medical device, including the laser fiber, as an entire unit into the patient toward the target stone and allow for suction of the stone through a working channel without blocking of the working channel by the laser fiber.

A target identification system for use in electrosurgery includes a probe, an optical splitter, and a spectroscopy system. The probe includes an optical pathway to pass a first optical signal to an anatomical target and at least a portion of a second optical signal from the anatomical target. The optical splitter includes a first port to direct the first optical signal to the optical pathway and to receive the at least a portion of the second optical signal from the optical pathway, a second port to receive the first optical signal, and a parabolic reflector to redirect the portion of the second optical signal. The spectroscopy system can identify a characteristic of the anatomical target based on the redirected at least a portion of the second optical signal.
