Abstract: An apparatus and method for removing an object from a tubular vessel are disclosed. The apparatus includes an expandable catheter and a plurality of wires configured to snare an object within a tubular vessel. A distal end of the expandable catheter may be manipulable between a first configuration wherein an opening thereon has a first diameter, and a second configuration wherein the opening has a second diameter which is larger than the first diameter. The apparatus may further include an inner catheter within its lumen, the inner catheter being extendable from the expandable catheter's lumen. The apparatus may additionally or alternatively include an outer catheter having a lumen from which the expandable catheter may be expanded.

Abstract: A catheter introducer system is provided with a catheter sheath having a valve providing for a friction fit around a catheter. The friction fit is selectively adjustable to vary between a first configuration for longitudinal movement of the catheter through the valve and a second configuration for holding the catheter longitudinally in place. The valve may includes a chuck to grip the catheter radially. The chuck may include an annular base and an opposed annular clamp, and a washer disposed between the base and the clamp. At least one of the base and the clamp may include a beveled surface that forces the washer radially inwardly as the base and the clamp are drawn together. The system also includes one or more dilators layered within the sheath. The layered dilators and sheaths are typically softer and more hydrophilic proceeding from the innermost dilator out to the sheath. The dilator may be reinforced by a wire embedded in the body of the dilator. The distal tip of the sheath may be stiffened by a ring affixed adjacent the distal end. The sheath may include in the innermost dilator a trocar formed of a flexible, hollow outer wire and an inner stiff needle, which can extend beyond the wire for piercing and be withdrawn to relax the wire for advancement of the dilator.

Abstract: A stent system is provided for percutaneous insertion in an artery of a main stent which includes at least one peripheral aperture defined through the stent wall. The stent system also includes a peripheral stent configured to be inserted into the peripheral aperture of the main stent. The peripheral stent extends, when inserted in the peripheral aperture, generally perpendicular to the longitudinal axis of the main stent. The stent system may further include a guidewire, insertable through the peripheral aperture, for maneuvering the main stent into place in the artery. The guidewire may be tapered toward its distal end. The stent system may also include a dilation device for dilating the peripheral stent within the peripheral aperture. The stent system may further include a tube inserted through the peripheral aperture of the main stent.

Abstract: A catheter apparatus is provided including an arterial catheter and a venous catheter wherein the distal ends of the catheter are insertable in an artery and a adjacent vein, respectively to a position adjacent a site for creating a fistula. The venous catheter includes a radially expandable structure adjacent the distal end which can be selectively extended outwardly to expand a portion of the wall of the vein adjacent the venous fistula site towards contact with the wall of the artery adjacent the arterial fistula site. The radially expandable structure of the venous catheter may include one or more balloons which can be inflated or a wire basket, or the vein can be expanded by injecting a fluid through an injection port into an isolated portion of the vein. The apparatus further includes a tool for creating an opening through the wall of the artery adjacent the arterial fistula site and an opening through the outwardly extending portion of the wall of the vein adjacent the venous fistula site to extend outwardly the portion of the wall of the vein towards contact with the wall of the artery. The tool may include a needle coupled to the arterial catheter adjacent the distal end of the arterial catheter, the needle being selectively switchable between a first, inactive configuration wherein the needle can be guided through the artery without causing trauma to the artery wall and a second, active configuration wherein the needle is operative to create the opening in the artery wall. The fistula between the openings may be completed with the aid of a stent, and the proximal portion of the vein blocked with an embolization device.
