An implantable vagal stimulation device with high-energy efficiency and novel data sensing is provided for use in a wide variety of applications where neural stimulation is required, including human heart rate control. The stimulation device uses low-impedance circuitry and digital waveforms to minimize energy losses, thereby requiring a relatively small battery. Front-loaded, passive filtering is employed to reduce electromagnetic noise sensitivity, leaving a clear physiological signal without degradations. This physiological signal is processed by a derivative zero transition detector (DZD), which is immune to variations in input signal dynamic range unlike traditional methods. Information that the DZD receives can be then interpreted and used along with an algorithm to execute appropriate vagal nerve stimulation.

In a subcutaneous implantable cardioverter/defibrillator, cardiac arrhythmias are detected to determine necessary therapeutic action. Cardiac signal information is sensed from far field electrodes implanted in a patient. The sensed cardiac signal information is then amplified and filtered. Parameters such as rate, QRS pulse width, cardiac QRS slew rate, amplitude and stability measures of these parameters from the filtered cardiac signal information are measured, processed and integrated to determine if the cardioverter/defibrillator needs to initiate therapeutic action.

Control of defibrillation therapy delivered by implantable medical devices (IMDs) using hemodynamic sensor feedback is disclosed. The hemodynamic sensor feedback allows for increased control over application of atrial defibrillation therapy. Specifically, the therapy is delivered when a fibrillation episode results in a discrete loss of hemodynamic function. Defibrillation therapy is thus withheld for hemodynamically benign arrhythmias.
