An electrocardiography patch is provided. A flexible backing is formed of an elongated strip of stretchable material and a pair of electrodes is respectively affixed on a contact surface of each end of the elongated strip. A flexible circuit is affixed on each end of the elongated strip and includes a pair of circuit traces electrically coupled to each electrode. A non-conductive receptacle is securely adhered on the one end of the elongated strip opposite the contact surface and formed to removably receive an electrocardiography monitor and house a battery. Electrical pads are provided on a bottom surface of the non-conductive receptacle to interface with electrical contacts protruding from the electrocardiography monitor. A pair of the electrical pads is formed to electrically couple with the electrodes. A pair of battery leads is formed on the non-conductive receptacle to electrically interface the battery to another pair of the electrical pads.

A computer-implemented method for evaluating ambulatory electrocardiographic (ECG) monitoring of cardiac rhythm disorders is provided. A patient is registered online and medical records for the patient are assembled. An ambulatory ECG monitor that includes leadless integrated sensing electrodes independently suspended from a flexible housing, is registered to the patient. An electrocardiogram is retrieved from the recording circuitry. The electrocardiogram and the medical records are evaluated against diagnostic criteria. Upon making a finding when the diagnostic criteria is met, the patient is referred to a cardiac rhythm specialist online, which includes sending the cardiac rhythm abnormality finding. As a result, both physicians and patients enjoy an ease-of-use not found with conventional ambulatory ECG monitors. By bypassing determining whether a referral is needed and separately establishing the referral, patients can be treated more completely and more rapidly than through conventional patient referral, especially for serious illness, with less cost and less delay.

A system and method for automated diagnosis of atrial fibrillation through remote monitoring is described. Physiological measures including data either recorded on a regular basis by a medical device or derived therefrom are stored. Physiological measures recorded during a baseline period are identified. Physiological measures including cardiac rhythm and changes to the cardiac rhythm originating subsequent to the baseline period are identified. Cardiac rhythm changes for palpitations are evaluated and a time course for the cardiac rhythm changes upon an indication of palpitations is determined. A patient status including an onset of atrial fibrillation conditioned on the time course comprising a short duration is formed.

Each patient care record is defined and includes collected measurements sets. Each collected measurements set is specified and includes individual measurements, which each relate to patient information recorded by a medical device adapted to be implanted. At least one of the collected measurements sets is processed. A set of the collected measurements regularly stored and periodically sent from the implantable medical device is received. The collected measurements set are stored into the patient care record. One or more of the collected measurements sets are analyzed in the patient care record for the individual patient relative to one or more other collected measurements sets stored in the patient care record. Feedback based on the analysis of the one or more collected measurements sets is sent.

Medical data provided by the patient is evaluated against the diagnostic criteria of each of the specialists for medical concerns. Each specialist in the referral tree corresponding to findings made under their respective diagnostic criteria is identified. The patient is referred for care to the specialist associated with the patient's general physician. Throughout, the patient is provided with information to make an informed decision with respect to the specialist care received.

A method for performing ambulatory electrocardiographic monitoring is provided. An ambulatory ECG monitor, that includes a plurality of sensing electrodes coupled to self-powered sensing circuitry, is provisioned. A monitoring site is located on the surface of a patient's chest at midline and above the body of the sternum adjacent to the fourth and fifth intercostal spaces. The electrodes are aligned and placed along the midline. The ambulatory ECG monitor is removably adhered to the monitoring site for the duration of monitoring. ECG data is sensed through the sensing electrodes at the monitoring site and the sensed ECG data is recorded into the sensing circuitry.
