Cardiac signal separation is employed to detect, monitor, track and/or trend ischemia using cardiac activation sequence information. Ischemia detection may involve sensing composite cardiac signals using implantable electrodes, and performing a signal separation that produces one or more cardiac activation signal vectors associated with one or more cardiac activation sequences. A change in the signal vector may be detected using subsequent separations. The change may be an elevation or depression of the ST segment of a cardiac cycle or other change indicative of myocardial ischemia, myocardial infarction, or other pathological change. The change may be used to predict, quantify, and/or qualify an event such as an arrhythmia, a myocardial infarction, or other pathologic change. Information associated with the vectors may be stored and used to track the vectors.

The defibrillation threshold represents an energy that when delivered at the first internal thoracic location creates an electric field strength in the target region of the heart that meets or exceeds the target electric field strength.

In an embodiment, the invention includes a method of calculating a pulmonary function parameter of a subject. The method can include obtaining a first signal indicative of lung volume change during breathing from a first sensor, obtaining a second signal indicative of distending pressure from a second sensor, and calculating the pulmonary function parameter based on the first signal and the second signal. In an embodiment, the invention includes a method of monitoring pulmonary or cardiac disease status. In an embodiment, the invention includes an implantable medical device. The implantable medical device can include a first sensor configured to produce a first signal indicative of lung volume change during breathing, a second sensor configured to produce a second signal indicative of intrapleural pressure, and a processor configured to calculate lung compliance or pulmonary resistance based on the first signal and the second signal.
