摘要: A retractor has a proximal portion comprising a first side portion having a first longitudinal edge and a second side portion having a second longitudinal edge. The first and second portions being movable relative to each other such that the first and second longitudinal edges can be positioned in close proximity to each other or spaced apart by a selected distance. A distal portion is coupled with the proximal portion. The distal portion has an outer surface and an inner surface partially defining a passage. The distal portion is capable of having a configuration when inserted within the patient wherein the cross-sectional area of the passage at a first location is greater than the cross-sectional area of the passage at a second location, wherein the first location is distal to the second location.

摘要: One or more shaped axial bore extending from an accessed posterior or anterior target point are formed in the cephalad direction through vertebral bodies and intervening discs, if present, in general alignment with a visualized, trans-sacral axial instrumentation/fusion (TASIF) line in a minimally invasive, low trauma, manner. An anterior axial instrumentation/fusion line (AAIFL) or a posterior axial instrumentation/fusion line (PAIFL) that extends from the anterior or posterior target point, respectively, in the cephalad direction following the spinal curvature through one or more vertebral body is visualized by radiographic or fluoroscopic equipment. Preferably, curved anterior or posterior TASIF axial bores are formed in axial or parallel or diverging alignment with the visualized AAIFL or PAIFL, respectively, employing bore forming tools that can be manipulated from proximal portions thereof that are located outside the patient's body to adjust the curvature of the anterior or posterior TASIF axial bores as they are formed in the cephalad direction. Further bore enlarging tools are employed to enlarge one or more selected section of the anterior or posterior TASIF axial bore(s), e.g., the cephalad bore end or a disc space, so as to provide a recess therein that can be employed for various purposes, e.g., to provide anchoring surfaces for spinal implants inserted into the anterior or posterior TASIF axial bore(s).

摘要: Apparatus for removing bone from a femoral notch, or the like, which includes a guidewire and a router assembly. The router assembly comprises a cutting head fixed to a shaft rotatably disposed in a hole through a body portion of a shield assembly. A hood portion of the shield assembly extends from the body portion of the shield assembly and covers a first portion of the cutting head while leaving exposed a second portion of the cutting head. The guidewire extends through a hole in the shaft, a hole in the cutting head, and a hole in the shield assembly hood portion, such that the router assembly is movable along the guidewire. The shaft is rotatable in the shield assembly body portion such that the second portion of the cutting head is engageable with the bone and operative to remove portions of the bone. The guidewire is installed in the knee so that it extends along the length to be occupied by a graft ligament. The guidewire and router assembly are flexible so that the bone-trimming operation may be conducted dynamically as the knee is moved through a range of natural motions.

摘要: A positioning tool for a joint socket cutting instrument or a implant is designed for use with a minimally invasive surgical procedure and in conjunction with a computer assisted surgical procedure. The positioning tool has a longitudinally extending drive shaft having a moveable joint at a first end and a drive coupling for connecting to a power source at a second end. A holder for mounting a cutting tool such a drill or as an acetabular cutting instrument or for mounting an acetabular implant is coupled to the moveable joint at the first end of the drive shaft for movement with respect to the drive shaft. The holder is rotatable about a central axis thereof when the drive shaft is rotated. The drive shaft includes a shaft bearing mounted thereon which is pivotally coupled to the shaft at a fixed longitudinal position and is pivotally coupled to a longitudinally extending first arm having a handle. A tracker system which is capable of being utilized by a computer-aided surgical system is mounted on the first arm. A second arm is provided which is pivotally connected to the holder at a first end and pivotally connected to the first arm at a second end. The resulting four bar linkage allows the holder and the cutting instrument/implant to be manipulated in any position while the known geometric relationship between the tracker and the holder allows the location of the holder to be displayed by the computer on a cathode ray tube with respect to a joint.

摘要: A cutting device (1) for cutting bone tissue (3), which is particularly intended for correcting malpositions, and which enables a segment of the bone tissue (3) to be cut along a predetermined cut surface (5). The cutting device (1) produces a high pressure fluid jet (2) which is guided by a control program stored in a control unit (4) to produce a cut surface (5) having a desired configuration, for example, a circular arc shape. The resulting bone parts (6, 7), created in this manner, can be fixed in a corrective position (8) with little effort and without significant loss of material. A collector (9), which is equipped with a supply line (14) for supplying a rinsing liquid and with a suction line (13), is provided for collecting the fluid from the fluid jet (2). Collector (9) simultaneously serves to reduce the energy remaining in the high-pressure fluid jet (2). A cut surface (5) having a freely selectable shape can be produced by the cutting device (1) and can be obtained without any difficulty by using automated handling systems. The cutting device of the invention enables a significant broadening of the field of use and of the design freedom for an orthopedic surgeon together with a use that preserves bone tissue.

摘要: The present invention is an apparatus for cutting allograft bone implants from donor bone. Die sets of standard sizes are installed in a press. Donor bone is then placed in position between the upper and the lower parts of a matched cutting set comprising a hollow cylindrical cutting blade, a mandrel, and a table which establishes a gap around the mandrel which guides the blade. After the bone to be shaped has been properly placed, a shaped cutting blade is driven through the donor bone, producing a precisely shaped allograft implant. The invention provides for rapid change out of blades, rapid cleaning, and easy maintenance.
