摘要: Biologically implantable prostheses and methods of using the same are disclosed. In one embodiment, a heart valve assembly includes a first annular prosthesis sized for introduction into a tissue annulus in a heart; a first magnet in the first prosthesis; a second valve prosthesis separate and disconnected from the first prosthesis and comprising a heart valve crown with leaflets, the second prosthesis configured for introduction into the tissue annulus after introducing the first prosthesis into the tissue annulus; and a second magnet in the second prosthesis, wherein an attraction between the first magnet and the second magnet engages the first prosthesis to the second prosthesis to attach the second prosthesis to the first prosthesis in the tissue annulus.

摘要: A method of using a prosthetic heart valve assembly can include implanting a first prosthesis directly into a native annulus. The first prosthesis can have a central longitudinal axis and a first engagement element. The method can also include engaging the first engagement element of the first prosthesis with a second engagement element of a second prosthesis to securely couple the second prosthesis to the first prosthesis. The second prosthesis can have a prosthetic valve. The first engagement element engages the second engagement element by moving the second prosthesis towards the first prosthesis in a direction generally parallel to the central longitudinal axis until the first engagement element engages the second engagement element.

摘要: A heart valve assembly includes a first annular prosthesis for implantation within a tissue annulus, a second valve prosthesis, and a plurality of magnets on the first and second prostheses to secure the second prosthesis to the first prosthesis. In one embodiment, the magnets are arranged to allow the second prosthesis to be secured to the first prosthesis in a predetermined angular orientation. During use, the first annular prosthesis is implanted into the annulus, and the second valve prosthesis is inserted into the annulus. The magnets orient the second prosthesis relative to the first prosthesis to align the second prosthesis with the first prosthesis in a predetermined angular orientation; and secure the second prosthesis to the first prosthesis in the predetermined angular orientation.

摘要: Devices, systems, and methods for suturing of body lumens allow the suturing of vascular puncture sites located at the distal end of a percutaneous tissue tract. An elongated articulated foot is inserted through a penetration and actuated extend the foot carrying suturing attachment cuffs. Needles are advanced from the shaft through the vessel wall outside of the penetration and into engagement with the needle cuffs after the foot has been drawn proximally up against the endothelial surface of the blood vessel. The cuffs lockingly engage the needles and can be withdrawn along the needle paths and through the tissue tract to form a loop of suture across the puncture. The articulating foot may be realigned with the shaft and withdrawn proximally through the tissue tract without dilating the tissue tract.

摘要: A method of closing a puncture in a blood vessel wall, including: positioning a suture placement device in a tissue tract and into a puncture in the blood vessel wall, the placement device having a length of suture with a first end, a second end and a bight between the first and second ends with the bight being prearranged on the device to define a pre-tied knot when the first end of the suture passes through the bight; advancing the first end of the length of suture in a distal direction from the device through a first penetration in the blood vessel wall adjacent to the puncture; guiding the suture length from the first penetration across a proximal side of the puncture to a second penetration adjacent to the puncture opposite the first penetration; and advancing the suture length in a proximal direction through the second penetration, through the device, and through the bight so that the two ends of the suture length are accessible external to the tissue tract for securing the suture across the puncture to close the puncture.

摘要: Devices, systems, and methods for suturing of body lumens allow the suturing of vascular puncture sites located at the distal end of a percutaneous tissue tract. An elongated articulated foot near a distal end of a shaft is inserted through the penetration and actuated so that the foot extends along the lumenal axis. The foot carries suturing attachment cuffs, and needles are advanced from the shaft through the vessel wall outside of the penetration and into engagement with the needle cuffs after the foot has been drawn proximally up against the endothelial surface of the blood vessel. The cross-section of the shaft within the tissue tract can be minimized by laterally deflecting the needles as they leave the shaft, while tapered depressions within the foot can guide the advancing needles into engagement with the cuffs. The cuffs lockingly engage the needles and can be withdrawn proximally along the needle paths and through the tissue tract so as to form a loop of suture across the puncture. The articulating foot may be realigned with the shaft and withdrawn proximally through the tissue tract without dilating the tissue tract. The suture may be provided with a bight between first and second ends, the bight including one or more loops prearranged to define a pre-tied knot when one or more ends of the suture past therethrough.

The fixturing device includes an elongate filament having first and second ends, and an enlarged fixturing device on the first end of the filament. The second end of the filament is held by the can by frictional resistance, the second end of the filament being removable from the can when a tensile force is applied between the needle and the second end of the filament that exceeds the frictional resistance. For example, the can may include a coil, corrugations, or a roughened inner surface to removably hold the end of the fixturing device.

摘要: A pneumatic pump having a variable buoyant actuator to substantially reduce the volume of a float, thereby increasing the fluid capacity for any pump of a given size. The pump includes an elongated housing, a sealable fluid entry aperture and a pipe communicating between an interior and an exterior of the housing. The housing includes a bottom wall and a cylindrical side wall extending from the bottom wall and terminating in an opening. A switchable valve control having a pod closes the opening. The pod has a plurality of valve seats and a plurality of corresponding valve elements, with the plurality of valve seats defining fluid inlet and output ports. The actuator is coupled to alternatingly placed valve elements in sealing engagement with fluid inlet and outlet ports in response to a level of fluid in the housing. The variable buoyant actuator includes first buoyant member disposed proximate to the bottom wall, and a buoyant amplifier disposed opposite to the first buoyant member, proximate to the pod. The first buoyant member is typically a cup or other similar receptacle, and the buoyant amplifier may be either an air-trap or a conventional float.
