A vessel sealing instrument having an instrument tip that comprises a pair of opposing clamping members movable to clamp a vessel to be sealed. The clamping members include an energy delivery structure capable of delivering localized radiofrequency (RF) electromagnetic (EM) energy and microwave EM energy separately or simultaneously into the vessel. The RF EM energy and the microwave EM energy are received in the energy delivery structure from a coaxial cable. Each energy delivery structure comprises first and second conductive elements separated by a planar dielectric layer. The first and second conductive elements are arranged at the opposed surface of the respective clamping member to act as active and return electrodes to transfer RF EM energy into biological tissue by conduction, and as an antenna to radiate microwave EM energy into biological tissue from the opposed surface.

Surgical cutting apparatus having a treatment channel and a measurement channel for conveying microwave energy from a source to an antenna at a cutting edge. The measurement channel operates at lower power than the treatment channel for determining when higher energy can be safely applied. The apparatus may deliver microwave radiation at differing frequencies to one or more antennas at the cutting edge, e.g. to provide different treatment effects. The source may generate an output for an antenna whose frequency can be selected e.g. for most efficient operation. Selection may be automatic based on detected magnitude and phase of reflected signals during a frequency sweep of a forward signal. Power delivered to tissue via the cutting element may be manually boosted to deal with large blood vessels. The apparatus may include a reflected power monitor for recognising behaviour in reflected signals received from the antenna to trigger automatic pre-emptive action.

The probe (116) receives reflected radiation from the tissue. The reflected radiation is delivered to a detector (178) along a second transmission path via a circulator (198) which isolates the forward radiation from the second transmission path. The detector has a input which is switchable between the reflected radiation from the second transmission path and a reference signal derived from the forward radiation, wherein detected magnitude and phase information of the reflected radiation to classify the tissue can be compensated for drift in magnitude and phase of the forward radiation by comparison with detected magnitude and phase information of the reference signal.

A surgical instrument (100) (e.g. scalpel) is disclosed which has an antenna arranged to emit a substantially uniform microwave radiation field (e.g. having a frequency of 5-100 GHz) at an edge of a cutting element (110) (e.g. blade). The emitted radiation can cauterise tissue e.g. broken blood vessels simultaneously with cutting. The antenna may be integral with the cutting element, e.g. a metallised piece of ceramic attachable at an end of a waveguide (120, 150) to receive radiation therefrom. The cutting element (110) can include a quarter wave transformer to couple power efficiently from the waveguide (120). The instrument can be used with impedance matching apparatus to control the energy delivered into the tissue. Also disclosed is an invasive ablation probe (e.g. insertable through a catheter) having a plurality of radiating elements whose emitted field combine to give a uniform effect at an insertion end of the probe.

An apparatus and method for ablating tissue is disclosed. The apparatus comprises a source of microwave radiation (1), a probe (5) for directing the microwave radiation into tissue, one or more detectors for detecting the power and phase of the microwave radiation and an impedance adjuster (50) for adjusting impedance so as to minimize the amount of microwave radiation which reflected back through the probe. The detector or detectors use a local oscillator (230) to derive the phase information. A modulator for modulating the microwave radiation to a cutting frequency is also disclosed.
