摘要: An electrosurgical system for radiofrequency ablation. An electrosurgical console includes a processor configured to determine impedance threshold values based on an initial tissue impedance value. The impedance threshold values may vary based on an elapsed time-based characteristic. A temperature set point is reduced based on the occurrence of the impedance event in which a tissue impedance value exceeds a threshold value. The reduced temperature set point may be fixed. The controller is configured to control the delivery of the RF energy drive the tissue temperature to the temperature set point(s). The temperature set point may not be reduced if the impedance event occurs during a ramp period, and an alert may be displayed. Secondary impedance threshold values may be utilized, and slower ramp periods may be provided for later in the ablation procedure. Methods of performing the ablation procedure with the electrosurgical system are also disclosed.

摘要: Apparatus and associated methods relate to controlling electrical power of an electrotherapeutic signal that is provided to a biological tissue engaged by an electrosurgical instrument during a medical procedure. Electrical power-a product of a voltage difference across and an electrical current conducted by the engaged biological tissue—is controlled according to a therapeutic schedule. The electrotherapeutic schedule can be reduced or terminated in response to a termination criterion being met. In some examples, the termination criterion is a current characteristic, such as, for example, a decrease in current conducted by the engaged biological tissue. In some examples, the termination criterion is a biological tissue resistance characteristic, such as, for example, an increase in the biological tissue resistance that exceeds a predetermined delta resistance value.

摘要: An ablation device and method for pulsed field ablation, the device comprising a catheter including an expandable basket, a set of electrodes formed on the expandable basket, and a pulse generator suitable for generating electric pulses wherein the pulse generator being in electrical connection with the set of electrodes. The expandable basket is formed of a braided mesh of filaments, wherein the filaments are made of nonconductive material, wherein at least portion of the filaments comprises a lumen, and wherein the lumen comprises melted material, wherein the filaments further include electrodes and conductive wires. The conductive wires at least partially lead inside of the lumen of the filaments, and are electrically connected to the electrodes.

摘要: A transseptal access system is disclosed. The system includes a radiofrequency perforation device having a proximal portion and a distal portion. The proximal portion has an electrically insulated outer surface, the distal portion is electrically conductive and terminates at a functional tip, and at least part of the distal portion is uninsulated. A tubular dilator is further disclosed comprising an elongated body terminating in a distal tip, a hub at the proximal end of the body, and a dilator lumen extending through the hub and the body. The dilator lumen is dimensioned to slidingly receive the radiofrequency perforation device. An electrical contact element disposed within the hub adjacent to the dilator lumen is also disclosed. When the radiofrequency perforation device is in a first position within the dilator lumen, the electrical contact element is in electrical engagement with the distal portion of the radiofrequency perforation device.

摘要: The present invention resides in one aspect in a device for monitoring luminal esophageal temperatures in a patient. The device includes a probe adapted to be inserted into an esophagus of the patient. The probe extends between a proximal end and a distal end. A first temperature sensor and a second temperature sensor are coupled to the probe. An electrode is also coupled to the probe. The second temperature sensor is displaced from the first temperature sensor along a longitudinal axis of the probe. A controller processes information received by the electrodes and the temperature sensors. The controller generates a live and continuously updating three dimensional anatomic map and three dimensional thermal map of the esophagus based at least in part on the information received from the temperature sensors and the electrodes. The thermal map and the anatomic map are displayed on a video monitor.

The power supply apparatus includes a processor configured to receive a stop command to cutoff the supply of the first electric energy and the second electric energy in a state in which the first electric energy and the second electric energy are supplied and determine whether or not the end effector is in contact with the biological tissue based on a parameter related to the first electric energy after receiving the stop command.

For example, this document describes methods and devices for delivering pulse-electric field electroporation for ablation in a selective and non-thermal means that has high specificity for tissue destruction while minimizing collateral damage. In one aspect, this document is directed to a catheter device that includes: a catheter shaft; a balloon member attached at a distal end portion of the catheter shaft; one or more electrodes on an outer surface of the balloon or in an interior of the balloon member and configured to deliver pulsed-electric field non-thermal electroporation ablation energy to treat ventricular fibrillation of a heart; and a control system configured to adjust a pressure within the balloon member.
