摘要: A method and instrumentation for spinal interbody fusion is disclosed. The instruments and methods are particularly adapted for interbody fusion from a posterior approach to the spine. One instrument is a retractor having a lockable pivotally mounted handle. Another instrument is a template for straddling the dura. A modular distractor is also provided and preferably includes a tapered shaft with a visualization window disposed therein. Yet another instrument is a depth gauge to verify bone opening depth and dimension, preferably including a radiopaque portion. A method contemplates the use of these instruments to prepare a disc space to receive an implant.

摘要: A guide sleeve with longitudinally offset bone engaging portions is provided for engagement with anterior-posterior offset vertebral bodies. In one aspect of the invention the guide tube has fixed bone engaging portions in a predetermined offset position. In another form of the invention, the guide sleeve is composed of at least two moveably connected portions adapted to permit adjustment of the offset between the bone engaging portions. Further, a method is provided for using a guide sleeve with offset bone engaging portions.

摘要: A retaining system for affixing a component to a vertebral body is disclosed. The system comprises a component and a retaining mechanism. The component comprises an inner surface configured to engage at least a portion of an endplate surface of the vertebral body, and an outer surface configured to engage the retaining mechanism, wherein the outer surface of the component comprises a first face directed substantially toward the vertebral body. The retaining mechanism comprises an inner surface configured to engage the outer surface of the component, and an outer surface configured to receive a fastener, wherein the inner surface of the retaining mechanism comprising a second face directed substantially away from the vertebral body.

摘要: A retention system for maintaining a screw to a vertebral plate. One or more screws extend through apertures within the vertebral plate. A cavity is positioned adjacent to and overlaps into the aperture. A ring is positioned within the cavity and held in position by a cap. The cap attaches to the plate to prevent removal of the ring. The ring is deflectable between a first shape to allow the screw to be inserted and removed from the aperture, and a second shape to prevent screw back-out from the aperture. A method of using the system is also included and comprises positioning the ring within a cavity in the plate, maintaining the position of the ring to the plate by attaching a cap, inserting a screw through the aperture and attaching the plate to a vertebral member, and positioning the ring over the screw head and preventing the screw from backing-out.

摘要: In one embodiment of the invention, an anterior fixation system includes a plate defining a plurality of screw holes, a number of screws and a number of locking assemblies for fixing the screws to the plate. The system includes two bone screws a fixed angle screw and a variable angle screw, that are configured to extend through the same screw openings in the fixation plate. The surgeon can select either the fixed or variable angled screws to be implanted with a single plate and can place either type of screw into any of the screw holes along the plate. The fixation plate according to the invention can include several screw holes in various patterns that provide the surgeon with great flexibility in the placement of bone screws depending upon the spinal anatomy and pathology. The invention further contemplates a locking assembly to lock one or more bone screws within a respective screw hole. In one embodiment, the locking assembly includes a washer that is held to the plate by a staked locking screw. The washer includes an outer surface that overlaps one or more screw holes. The washer is initially loosely held to the plate by the locking screw so that various tools and bone screws can be passed through the screw holes. In one embodiment of the washer, the washer includes cut-outs corresponding to the screw holes, along with a notch and key configuration for setting the locking washer in its locked configuration.

摘要: One embodiment of a hollow spinal spacer (10) includes a curved anterior wall (11) having opposite ends (12, 13), a posterior wall (15) having opposite ends (16, 17), two lateral walls (20, 21), each integrally connected between the opposite ends (12, 13, 16, 17) of the anterior (11) and posterior (15) walls to define a chamber (30). The walls (11, 15, 20, 21) include a superior face (35) and an inferior face (40). The superior face (35) defines a first opening (36) in communication with the chamber (30) and includes a first vertebral engaging surface (37). The inferior face (40) defines a second opening (41) in communication with the chamber (30) and includes a second vertebral engaging surface (42). In one embodiment, a lateral wing projects from an external surface of each of the lateral walls. In another embodiment, the anterior wall defines an anterior superior surface and an anterior inferior surface. The anterior superior surface has a concave shape defining a first radius which is configured to correspond to the anterior shape of an inferior vertebral endplate. The anterior inferior surface has a convex shape defining a second radius which is configured to correspond to the anterior shape of a superior vertebral endplate. In other embodiments, the lateral walls define a convex lateral superior surface and a convex lateral inferior surface. Each of the lateral surfaces define a radius configured to correspond to the shape of a vertebral endplate.

摘要: An anterior fixation system includes a plate defining a plurality of screw holes, a number of screws and a number of locking assemblies for fixing the screws to the plate. The system includes two bone screws, a fixed angle screw and a variable angle screw, that are configured to extend through the same screw openings in the fixation plate. The surgeon can select either the fixed or variable angled screws to be implanted with a single plate and can place either type of screw into any of the screw holes along the plate.
