摘要: The present invention provides endoscopic instrumentation and surgical techniques especially useful for accessing and removing at least a portion of an intervertebral disc. Instrumentation includes tissue spreaders and cutting instruments. In particular, the tissue spreading instruments include instruments for spreading pre-sacral tissue such as fascia and for spreading adjacent vertebrae to facilitate access to the intervertebral disc. Endoscopic cutting instruments deliver a cutting blade in a sheathed position to the site of the disc nucleus followed by deployment to remove portions of the disc nucleus.

The apparatus include an anchoring device having an elongated suture with an anchor at each end thereof and a cinching member therebetween for adjusting the operative length of the suture, an articulating apparatus for applying the anchoring device during an endoscopic or laparoscopic procedure, and an apparatus for drawing the suture relative to the cinching member after it has been emplaced.

摘要: A self contained gas powered endoscopic surgical apparatus is provided for placing lateral lines of surgical fasteners into body tissue. The apparatus includes an anvil member and a surgical fastener cartridge member mounted to the distal end of an elongated endoscopic portion. A tubular collar of the endoscopic portion moves distally to engage the anvil member and bias the anvil member and the cartridge member into cooperative alignment, thereby clamping body tissue to be fastened between the anvil member and the cartridge member. A self contained pneumatic system is disposed in the surgical apparatus and is actuable to eject and/or form the surgical fasteners in the clamped body tissue. The apparatus further comprises a locking mechanism for preventing firing of the instrument after it has been fired a predetermined number of times. A counter for displaying the number of times the apparatus has been fired may be provided. A firing interlock is also shown which prevents accidental actuation of the firing trigger. A clamping interlock is shown which prevents approximation of the jaws when the jaws are either misaligned or improperly inserted into the instrument.

The cutting instrument consists of a tubular sheath or obturator constructed of a transparent material which is connected to a handle which includes means for securing a viewing instrument such as an endoscope thereto. A cutting blade is moved from a first protected or enclosed position to a second armed position, where only the blade edge is exposed at an opening in the obturator.

The invention includes an anchoring device having an elongated suture with an anchor at each end thereof and a cinching member therebetween for adjusting the length of the suture, an apparatus for applying the anchoring device, and an apparatus for drawing the suture relative to the cinching member after it has been emplaced. The method involves securing a first anchor to a first structure, such as the vaginal wall, securing a second anchor to a second structure, such as Cooper's ligament, and drawing the suture relative to the cinching member to approximate the first and second anchors, and thereby move the first structure and the second structure into juxtaposition so as to approximate them.

摘要: A method for percutaneous dissection of a segment of vascular tissue from a patient is provided which includes providing an elongated tubular member having a proximal end and a distal end, and blunt dissection structure positioned adjacent the distal end of the tubular member and remotely actuable from adjacent the proximal end of the tubular member. The method further provides for percutaneously accessing a first end portion of the vascular tissue segment, the blunt dissection structure is positioned adjacent the vascular tissue segment and is actuated to bluntly dissect the vascular tissue segment from surrounding body tissue.
