摘要: A hydraulically actuated medical instrument includes an elongated shaft having proximal and distal ends, a hydraulically actuated end effector at the distal end of the shaft and a fluid flow path extending through the shaft to the end effector. The shaft may be capable of an angled configuration where at least a portion of the shaft is manually movable to a direction in which it extends at an angle to another portion of the shaft. The end effector is not limited to a linear configuration and at least a portion of an elongated end effector extends at an angle relative to another portion of the end effector as is needed or desirable for the particular procedure to be performed.

摘要: The methods and apparatus for lead placement on a surface of the heart are employed using an elongated body having proximal and distal end portions. The body defines a lead receiving passageway extending between a proximal inlet and a distal outlet for receiving a lead therethrough for contact with the heart surface. The elongated body is adapted for insertion between a pericardium and an epicardial surface. At least a portion of the body may have a non-circular cross-sectional shape adapted to retain the body orientation between the pericardium and the epicardial surface.

摘要: An actuator mechanism for an endoscopic instrument includes a stationary handle having a stepped throughbore with a relatively smaller diameter at the distal end and a relatively larger diameter at the proximal end. A cylindrical rack member is slidably disposed in the larger diameter portion of the throughbore. The larger diameter portion of the throughbore has a lower slot opening, and a movable lever is pivotally coupled to the stationary handle and is provided with a toothed upper pinion which enters the slot in the larger diameter portion of the throughbore and engages the rack member. The proximal end of the tube member is coupled to the distal smaller diameter portion of the throughbore and the proximal end of the control member is coupled to the distal end of the rack member.

摘要: An ophthalmic device that includes a mask configured to be implanted posterior of the cornea. The mask is configured to increase the depth of focus of the patient. The mask includes an aperture configured to transmit along an optical axis substantially all incident light and a substantially opaque portion surrounding at least a portion of the aperture. The ophthalmic device can further include a plurality of light transmission structures configured to allow some light to pass through the substantially opaque portion. The light transmission structures are configured to minimize generation of visible artifacts due to the transmission of light through the light transmission structures.

摘要: A method is provided for increasing the depth of focus of an eye of a patient. A visual axis of the eye is aligned with an instrument axis of an ophthalmic instrument. The ophthalmic instrument has an aperture through which the patient may look along the instrument axis. A first reference target is imaged on the instrument axis at a first distance with respect to the eye. A second reference target is imaged on the instrument axis with the ophthalmic instrument at a second distance with respect to the eye. The second distance is greater than the first distance. Movement is provided such that the patient's eye is in a position where the images of the first and second reference targets appear to the patient's eye to be aligned. A mask comprising a pin-hole aperture having a mask axis is aligned with the instrument axis such that the mask axis and the instrument axis are substantially collinear. The mask is applied to the eye of the patient while the alignment of the mask axis and the instrument axis is maintained. Maintaining alignment of the mask axis and the instrument axis may be facilitated by capturing an image of the eye.

摘要: An electrocautery probe includes a distal loop electrode mounted between a pair of arms which are joined at their proximal ends to an electrode lead, and a mounting sleeve for slideably coupling the probe to the guide tube of a resectoscope. The arms are skewed from a longitudinal axis defined by the electrode lead and are covered with an insulative material which extends to the distal end of the arms and terminates in a straight edge parallel to a plane substantially perpendicular to the arms. The loop electrode is angled approximately 25.degree.-35.degree. proximally relative to a plane substantially perpendicular to the electrode lead. The loop electrode further defines sharp upper distal and proximal edges. A cross-section of the electrode further defines an upper surface, a leading distal surface, a lower distal surface, a lower proximal surface.

摘要: An electrocautery probe includes a pair of arms between which a roller electrode is rotatably mounted. The arms are joined at their proximal ends to an electrode lead and a mounting sleeve is provided intermediate of the arms and the lead for slideably coupling the probe to a resectoscope. According to one aspect of the invention, the roller electrode includes a cylindrical member having a central bore and an outer surface, and an insert extending longitudinally through the central bore. Preferably the cylindrical member is made of a first conductive material and the insert is made of a second conductive material. The insert has a plurality of corners and an axial bore. The roller electrode is preferably made of stainless steel, while the insert is preferably made of copper or tungsten. The outer diameter of the roller electrode is preferably approximately 0.120 inches, while the wall of the distance from at least on of the corners of the insert to the outer surface of cylindrical member is preferably approximately 0.005 inches thick. The cross-section of the central bore and the insert may similarly define any polygonal or other shape having corners. It has been discovered that the corners of the insert focus the cautery energy through the roller electrode to provide high heat zones for enhanced tissue vaporization.

摘要: An arthroscopic surgical instrument includes a surgical punch end effector assembly, a hollow tube defining a fluid passageway, a push rod extending through the hollow tube, an actuating lever coupled to a proximal end of the push rod, and a fluid coupler extending into the hollow tube and coupling to a vacuum source which applies suction through the fluid passageway. The end effector assembly has movable punch and stationary die end effectors. The stationary die end effector includes separate upper die and lower cup portions which are coupled to the distal end of the hollow tube, and which are fixed together during an assembly procedure. The upper die portion has a groove into which a tongue of the movable end effector is fitted prior to fixing the separate portions of the stationary end effector. When the stationary die end effector is assembled with the movable punch end effector pivoting therein, and the end effectors are in a closed position, a substantially closed cavity or hollow is formed in the end effectors with the cavity having a proximal opening to the tube. Tissue punched by the movable end effector falls into the cavity, and due to the fact that the cavity is substantially closed, the suction extends into the cavity and suctions the tissue out through the fluid passageway.
