    摘要: A device for surgical implantation to replace damaged tissue in a joint (such as a meniscus in a knee) is created from a hydrogel that is reinforced by a three-dimensional flexible fibrous mesh. In a meniscal implant, the mesh is exposed at one or more locations around the periphery, to provide anchoring attachments that can be sutured, pinned, or otherwise securely affixed to tissue that surrounds the implant. The fibrous mesh should extend throughout most of the thickness of the hydrogel, to create an “interpenetrating network” (IPN) of fibers modelled after certain types of natural body tissues. Articulating surfaces which will rub and slide against cartilage should be coated with a hydrogel layer that is completely smooth and nonabrasive, and made of a material that remains constantly wet. This composite structure provides a meniscal implant with improved strength, performance, and wettability compared to implants of the prior art. This type of implant may also be useful in repairing other joints, such as shoulders, wrists, ankles, or elbows, and in repairing injured or diseased hands, fingers, feet, or toes.

    摘要: A novel fixation system for fixing a graft ligament in a bone tunnel. The fixation system comprises an interference screw comprising a body having a distal end and a proximal end, screw threads extending longitudinally along the body, and a transversely-extending region formed in the body for receiving a transverse pin therein, whereby to securely lock the interference screw, and hence the graft ligament, to the bone. In accordance with a further feature of the present invention, there is provided a method for attaching a graft ligament to a bone, the method comprising the steps of: (i) drilling a tunnel in the bone; (ii) positioning the graft ligament in the bone tunnel; (iii) placing an interference screw in the bone tunnel so as to force the graft ligament laterally against the opposite side of the bone tunnel; and (iv) advancing a transverse pin transversely through the bone and through the interference screw so as to securely lock the interference screw, and hence the graft ligament, to the bone. The present invention can also be applied to attach other objects to bone, e.g., a bone fragment to bone.

    摘要: A simple and flexible artificial ligament which easily conforms to a patient's anatomy and can be used independently or in combination with an intervertebral graft, implant or prosthesis to return stability to the spine subsequent to a surgical spinal procedure is disclosed. In one preferred embodiment, the artificial ligament is in the form of a flexible conformable plate dimensioned to span adjacent vertebrae and having openings for reception of bone screws, fasteners, etc. to mount the plate to the vertebrae. The biomechanical supporting characteristics of the plate approximate the characteristics of the ligament (e.g., anterior spinal) which it replaces thereby providing appropriate support to the spine in extension which also permitting normal spine mobility. A method of supporting adjacent vertebrae with the artificial ligament is also disclosed.

    摘要: An anchoring apparatus and method of use are disclosed that secure a tendon or ligament within an interior opening of a bone structure is disclosed. The apparatus comprises a securing device, a retention device, and an anchoring device. The securing device has an elongated body with an outer surface and an inner surface wherein the tendon or ligament fits between the interior of the open bone structure and the outer surface of the securing device. The retention device has an elongated body and a retention head, wherein the retention device securely fits within the interior surface of the securing device. The anchoring or holding device connects to the retention head of the retention device and extends outside the interior opening of the bone structure to engage the outer surface of the bone structure to hold the retention device and securing device in a fixed position inside the bone structure.

    摘要: A method for the reconstruction of the anterior cruciate ligament of a patient is described. The process comprises the steps of: 1) harvesting from the patient semitendonosus and gracilis tendons, or other suitable tendons of adequate length, to form a tendon graft; 2) forming a hole through the patient's femur from the intercondylar notch therein anteriorly and laterally; 3) enlarging the cross-sectional area of that femoral hole adjacent the notch sufficiently to receive one end of the tendon graft and a suitable screw; 4) forming a suitably positioned hole through the patient's tibia opening at one end adjacent the medial tibial spine and having a cross-sectional area sufficient to receive the other end of the tendon graft and a suitable screw; 5) drawing the one end of the tendon graft through the tibial hole and the joint into the femoral hole while leaving the other end of the tendon graft in the tibial hole; 6) inserting a suitable screw into the femoral hole from its intercondylar notch end until the head of the screw is just within that end of the hole and the screw is pressing the tendon graft directly and firmly against a sidewall of the femoral hole; and 7) after tensioning the tendon graft, inserting a suitable screw into the hole in the tibia externally until the head of the screw is within the exterior end of the tibial hole and the screw is pressing the tendon graft directly and firmly against a sidewall of the tibial hole.
