摘要: A radially expandable sheath is provided having a substantially closed distal end with at least two sidewalls extending proximally therefrom and defining a central lumen. Each sidewall can have a substantially concave outer surface adapted to seat a graft member, and each side wall is at least partially separated by a longitudinally oriented slot that extends from a proximal end along a substantial length of each sidewall. The slot preferably terminates at a position just proximal to the distal end. The device can also include a sheath expander that is adapted to be disposed in the central lumen of the radially expandable sheath and that is configured to flex the sidewalls to radially expand the sheath so as to fix a graft member within a bone tunnel.

摘要: The instant disclosure is directed to methods of improving bone-soft tissue healing using biocompatible electrospun polymer fibers. In one embodiment, a method may include locating a portion of a subject's bone, affixing a tendon or ligament to the bone using a hardware fixture, and placing a patch comprising at least one electrospun polymer fiber in physical communication with both the bone and the tendon or ligament. In some embodiments, the bone may be a humerus, and the tendon or ligament may be a supraspinatus tendon. In certain embodiments, the patch may comprise substantially parallel electrospun polymer fibers, and may be placed such that the fibers are also substantially parallel with the long axis of the tendon or ligament.

摘要: Methods and devices are provided for anchoring a ligament or tendon to bone. In one embodiment, a surgical implant is provided having a sheath and an expander that is received within the sheath. Various delivery tools, including a sheath inserter and a driver, are also provided. In use, the sheath inserter can be used to position a tendon within a prepared bone hole, and it can be used to deliver the sheath with a guidewire coupled thereto into the bone hole. The driver can be provided for delivering the expander into the sheath. A loader can optionally be used to load the driver and expander onto the guidewire coupled to the implanted sheath.

摘要: Described herein are methods and apparatuses for use in repair of a patient's tissue by connecting the tissue to the bone using a transosteal tunnel and anchor configured to pass through the transosteal tunnel. In particular, described herein are methods of repairing an anterior cruciate ligament (ACL) and torn meniscal root. These anchoring apparatuses and method of using them are particularly well suited for use with the low-profile suture passers described herein, since these suture passers may allow access to previously inaccessible regions of the knee (or other body regions).

摘要: Tissue anchors and systems and methods for delivering and removing them are provided. In an exemplary embodiment, the tissue anchor may include an outer shell including a proximal end, a distal end, a passage extending therebetween, and a plurality of expandable arms; an expander including a proximal portion disposed within the passage and distal portion that extends distally from the outer shell distal, the expander movable proximally relative to the outer shell to a deployed position wherein the distal portion directs the plurality of arms outwardly to engage adjacent bone. A fork extends from the distal portion of the expander including a pair of tines spaced apart from one another and a concave distal surface extending between the tines. A suture loop extends from the distal portion of the expander for capturing a tissue structure.

The system for fixing soft tissue within a bone tunnel comprising a first fixation member having a proximal end and a distal end and a bore extending from said proximal end to said distal end, said first fixation member is adapted for insertion against a first portion of a soft tissue positioned within a bone tunnel, a second portion of said soft tissue emerging from said bone tunnel; a second fixation member adapted to engage said bore of said first fixation member, having means for restraining disengagement therewith and a proximal end; and a third fixation member adapted to engage said second portion of said soft tissue, having means for coupling onto said proximal end of said second fixation member and restraining disengagement therewith.

摘要: Surgical devices and methods are provided for anchoring tissue to bone, and more particularly methods and devices are provided for preventing over-insertion of an expander into a sheath of a two-piece anchor. For example, a tendon anchoring system is provided with an anchor assembly and an inserter tool. The anchor assembly includes a sheath with a threaded lumen and a threaded expander screw configured to be threadably disposed within the sheath to cause the sheath to expand outward. The inserter tool includes an elongate outer shaft with a distal end configured to couple to a proximal end of the sheath, and an elongate inner shaft with a distal drive tip configured to engage a proximal end of the expander screw. In one embodiment, the inner shaft can be rotatable relative to the outer shaft to thread the expander screw into the sheath, and the inner shaft can be prevented from rotating relative to the outer shaft when the expander screw is fully threaded into the sheath to prevent over-insertion of the expander screw into the sheath.

摘要: The present disclosure relates, in some aspects, to orthopedic implants for securing soft tissue to bone and methods for using the same. One particular implant comprises a first exposed porous surface region, having pores for promoting bone ingrowth, and a second exposed porous surface, having pores for promoting soft tissue ingrowth. At least some of the pores of the first exposed porous surface region may be seeded with osteocytic factors and at least some of the pores of the second exposed porous surface region may be seeded with fibrocytic factors. Such orthopedic implants can advantageously facilitate regeneration of the soft tissue to bone interface.

摘要: Techniques and reconstruction systems for soft tissue surgical repairs. The biceps tenodesis technique of the present invention provides fixation of a single diameter of the biceps tendon in a socket without the need for externalizing the tendon and without the need to pass any suture (or similar material) through any portion of the tendon (i.e., without the need to stitch or whipstitch any area/portion of the tendon). The technique improves the biomechanics of the combined fixation and helps overcome surgeons' concerns about rapid return to ADLs.
