摘要: Devices and methods for fixating a graft tendon in a bone tunnel for a ligament reconstruction or repair procedure are provided. In general, the described implantable tissue fixation device includes first and second elongate, substantially rigid support members that are discrete elements separated from each other, and at least one flexible member connecting the rigid support members. The tissue fixation device can have changeable dimensions such that it has at least one dimension that is smaller in a delivery configuration than in a deployed configuration. In both delivery and deployed configurations, the first and second rigid support members are positioned in a non-intersecting orientation with respect to one another. A graft retention loop coupled to the tissue fixation device has the graft tendon coupled thereto and extending into the bone tunnel so as affix the graft into the bone.

摘要: Devices and methods for joining a first and second tissue in a patient are disclosed that include a base with a plurality of recurved tines oriented to a tine axis and extending from a first surface of the base. The tines provide unidirectional traction of the first tissue along the tine axis toward the first surface. The first tissue is secured to the first surface of the device at the plurality of recurved tines and the second tissue is secured to the device at a second surface opposite the first surface to join the first and second tissues.

摘要: A fixation apparatus is provided that includes a reception portion on a proximal end of the fixation apparatus. The reception portion couples the fixation apparatus to a tension element, and adjustment of the tension element with respect to the fixation apparatus does not cause a change in a position of the fixation apparatus. The fixation apparatus also includes a chamber in a distal portion of the fixation device. The chamber provides a supportive enclosure for a tension element that runs through the chamber and is anchored at the reception portion, and a force vector of the tension element exiting at the distal end of the fixation device is redirected at an axis independent of the axis of the fixation apparatus from the proximal end to the distal end.

摘要: A graft tensioning tool includes an elongate frame having a proximal end, a distal end, and an axial passage extending therethrough. The distal end is configured to detachably engage a tunnel ring at an entrance to a tunnel in bone, and the frame includes a first suture puller and a second suture puller. Each suture puller detachably engages a length of suture emerging through the attached to suture and/or a soft tissue implant in the bone tunnel. A tensioner on the elongate frame displaces the suture pullers in a proximal direction to apply a controlled tension to the suture lengths and/or implant. A tension gauge measures tension applied to the suture pullers, and a locking tool is configured to engage the suture lock through the axial passage of the frame, allowing a user to fix the suture ends to bone when a target tension is attained.

摘要: The invention discloses a modular surgical bone screw assembly having a threaded base portion and a screw head adapted to attach a variety of structures to bone. The features of the screw include a modular design for the assembly that separates the function of anchorage to the recipient bone to one of the modules of the device (threaded portion) from the other function of attaching another element (such as bone block, membrane, orthodontic wires, elastics etc) to a second module (the screw head). The design also allows the head to be exchanged after the implant has been placed.

摘要: Devices and methods for soft tissue reconstruction are provided. One exemplary embodiment of a device includes a body a suture filament extending through through-holes formed in the body. The suture is formed into an overhand knot having three collapsible openings with filament limbs extending from the knot. The limbs can be passed through the thru-holes of the body and selectively into the collapsible openings of the overhand knot to form coils extending to an opposite side of the body. For example, one limb can be passed through first and second collapsible openings to form two coils, while the other limb can be passed through the first and third collapsible openings to form two coils. The collapsible openings are collapsed around the limbs disposed in the openings, resulting in a secured overhand knot. Other configurations of knots, as well as devices and methods for use in tissue reconstruction, are also provided.

摘要: Devices, systems and/or methods for repairing soft tissue adjacent a soft tissue repair site. In one embodiment, a repair device includes an anchor member, a capture member and one or more flexible members. The anchor member includes a base with at least four legs extending integrally from the base, the at least four legs configured to be moveable from a straight first position to a formed second position. The capture member is configured to be coupled to the anchor member such that the at least four legs of the anchor member move around structural portions of the capture member with the at least four legs of the anchor member in the formed second position. The one or more flexible members are coupled to the base of the anchor member, the one or more flexible members extending at least partially along the base of the anchor member.

摘要: Tissue anchors and systems and methods for delivering and removing them are provided. In an exemplary embodiment, the tissue anchor may include an outer shell including a proximal end, a distal end, a passage extending therebetween, and a plurality of expandable arms; an expander including a proximal portion disposed within the passage and distal portion that extends distally from the outer shell distal, the expander movable proximally relative to the outer shell to a deployed position wherein the distal portion directs the plurality of arms outwardly to engage adjacent bone. A fork extends from the distal portion of the expander including a pair of tines spaced apart from one another and a concave distal surface extending between the tines. A suture loop extends from the distal portion of the expander for capturing a tissue structure.

摘要: An improved device provides a cost-effective, time-effective way to stably and reliably affix soft tissues (e.g., tendon or ligament) to the surface of anatomical structures with comparable or greater strength and stiffness (e.g., bone). The device does not require specialized procedures or tools. A single device applies both incremental tensioning and compression to the soft tissue being affixed. The device allows the user, typically a physician or veterinarian, to reliably manipulate, control, and maintain tissue tension. The device is versatile, and can accommodate attachment of soft tissues of different sizes and mechanical properties to bone or to other soft tissues. These features provide flexibility to human and veterinary health practitioners in a variety of specialized areas including orthopedics, joint stabilization, soft tissue repair, spinal surgery, and plastic surgery.

摘要: Systems and methods for providing fixation of soft tissue (for example, ligament) to bone by employing a novel multi-piece device. The novel multi-piece device is a ligament fixation system consisting of a larger collar that is initially impacted into a bone aperture, and a smaller plug/bullet-shaped part that is impacted into the larger collar. During ligament repair/reconstruction, the graft is inserted into a bone tunnel with sutures from the graft extending outside the tunnel. The sutures are passed through a collar of the multi-part device and the collar is then inserted into the bone tunnel. The graft suture strands may be tensioned. Subsequently, the smaller plug/bullet-shaped part is impacted into the collar thereby maintaining the tension on the graft sutures. The plug/bullet-shaped part is tightened after the desired tension on the suture is achieved.
