A device (310") for applying a compressive force to the uterus, the device comprising a contact portion (312") for contacting the uterus and providing a compressive force thereto, and at least two elongate handles (314a", 314b") each extending from the contact portion along a respective longitudinal axis (100a, 100b). The at least two elongate handles are configured to facilitate insertion of the device into the body and handling of the device from outside the body when inserted. At least a part of the contact portion is configured to be radially moveable relative to the longitudinal axes between a first radial position and a second radial position where the second radial position is radially outward of the first radial position.

A bipolar electrosurgical colpotomy device includes a colpotomy cup (10, 10C) and a tissue containment bag (20, 220). The colpotomy cup (10, 10C) includes a first electrode (31, 76B, 76C) located adjacent to a rim (30, 55C) of the cup, the rim being configured to contact an internal surface of a patient's cervico-vaginal junction. The tissue containment bag (20, 220) includes an opening (50) and a second electrode (51, 251) located adjacent to the opening. The tissue containment bag is configured to surround the patient's uterus and the opening is configured to fit over an external surface of the patient's cervico-vaginal junction. Applying a voltage potential between the first and second electrodes vaporizes tissue at the patient's cervico-vaginal junction by electrosurgical energy passing through the tissue disposed between the first and second electrodes. The tissue containment bag then can be used to remove the uterus from the patient, possibly after masticating the uterus within the bag.

A general uterine manipulator 10 incorporates an elongated hollow tube 12 defining an internal passage 14. Tube 12 has opposite first and second ends 16 and 18, formed with internal threads Tl and T2; and a smooth continuous outer surface 20 of constant outer diameter. A first fitting comprising a tail screw 22 is screwed into thread Tl and a second fitting cervical screw 24 is screwed in thread T2. An inner manipulator rod 26 extends through the first fitting 22, tube 12, and second fitting 24. A range of alternate first and second fitting is provided. A forceps holder 70 is attachable to the tube 12 between ends 16 and 18. The manipulator 10 may also support a cervical funnel 90 and a plug 92.

The object of the invention is a reusable medical instrument for controlled uterine manipulation during gynaecologic endoscopic operative procedures. The instrument provides a simple and safe uterine manipulation and can be adapted to anatomy of a patient. The object of the invention, consists of four pieces which can easily be put together: a guiding rod (1), a cylinder with a cervical cup (2), a cylindrical locking nut (9) a pear shaped nut (3) for occlusion of lower vagina. A cylinder with a cervical cup (2) and a pear shaped nuts (3,9) are made in three dimensions and are interchangeable to better adapt to anatomy of a patient.

The present disclosure relates to a uterine manipulator which comprises a handle portion, an arm portion, having a proximal end connected to the handle portion and a distal end, and a body portion connected to the distal end of the arm portion. The body portion comprises at a distal end, a hollow uterine tip adapted to be inserted into a patient's uterine cavity via the cervix in use, and at a proximal end, a fitting connector. The body portion further comprises an internal passage providing fluid communication between the fitting connector and the hollow uterine tip. The present disclosure also relates to a vaginal cup for delineation of a patient's cervico-vaginal junction. The vaginal cup is adapted for removable attachment to the body portion of the uterine manipulator, and is of a geometry to prevent loss of pneumoperitoneum following colpotomy. The vaginal cup has a wall defining an opening, a base end and an outer end, where the opening is at the outer end, which is closest to the hollow uterine tip when the vaginal cup is attached to the body portion.

An apparatus for use in connection with an endoscopic surgery comprises an elongated body adapted to be placed at least partially in the vagina of a female patient partially passing through the wall of the vagina such that an end portion of said elongated element is introduced into rectouterine pouch; the elongated body comprises at least an attachment means for accommodating at least a part of at least a surgical instrument comprising an end portion adapted to be received inside the vagina, the surgical instrument and the apparatus are movable to each other. An endobag having an elongated port is provided through the vagina into the pelvis. A method for use in a pelvic or peritoneal surgery is provided wherein at least an elongated end portion of a surgical instrument is provided in the cavity of the uterus through the vagina of a female patient such that the uterus can be repositioned by manipulating the surgical instrument and an incision is performed on the vaginal wall for accessing to the rectouterine pouch, a flexible element is provided in pelvis wherein the flexible element is looped around the uterus such that the flexible element encircles the uterus.

A surgical instrument system is provided for a hand-assisted, laparoscopic procedure. The system has a laparoscopic disc that is adaptable to hold multiple surgical instrument. Other elements in the surgical instrument system include a storage pouch for placement within the body cavity, a device for supporting tissue, a device for relocating tissue away from a surgical site, and tissue markers for marking tissue to plan a surgical event.
