摘要:
An intramedullary nail (10) comprises a unitary piece of elongate material in which the proximal portion (12) and distal portion (16) are substantially greater in thickness than the intermediate portion (14). The nail (10) is provided with a longitudinal slot (50) and transverse openings (24), (46) through the proximal portion (12) and distal portion (16) to receive interlocking screws, and threads (26) are provided for attachment of insertion and extraction devices. The invention further includes an advantageous method of making the nail from a single length of material, such as stainless steel tubing. The advantageous method includes a two-step process of machining the intermediate portion of the length of tubing to reduce the outside diameter (54) and wall thickness (58) thereof and swaging either the proximal portion (12) or the distal portion (16) (or both) to reduce the inside and outside diameters thereof. Another aspect of the invention is the provision of a cap (260) for the internally threaded end (26) of the proximal portion (12) to prevent the ingrowth of tissue.

A variable length fixation device (10) for insertion into a hole formed in two or more bone fragments has a barrel portion (11) and a fastener element (30). The barrel portion (11) is secured to an inside surface of the hole in a proximal one of the bone fragments. The fastener element (30) is telescopically mounted to the barrel portion (11) and is extendable into a distal one of the bone fragments. The assembly prevents lateral movements of the distal fragment, relative to the proximal fragment, while allowing axial relative movements (i.e., linear movements along the longitudinal axis of the fastener element) to occur. In a preferred embodiment, the fastener element (30) is a cannulated bone screw having a hex-shaped cross-section (42) on one end which is telescopically received into a hex-shaped interior (16) of the hollow barrel portion (11). The two components are preferably permanently joined to form a one-piece assembly having a variable overall length. The fixation device of the present invention is substantially insertable in its entirety into the hole which extends across the fracture site, and may be installed with a minimal amount of trauma and damage to surrounding bone and soft tissues.

This invention relates to a floating bearing knee joint prosthesis (146) having a fixed tibial post used for replacing the articulating knee portion of a femur and a tibia. The floating bearing knee joint prosthesis (146) includes a femoral component (152), a tibial component (154), a guide post (206) and a bearing member (156). The femoral component (152) includes an engagement member (176), a first femoral bearing surface (162) and a second femoral bearing surface (164). The tibial component (154) includes a tibial bearing surface (196). The guide post (206) extends from the tibial component (154) and is operable to be engaged by the engagement member (176) of the femoral component (152). The bearing member (156) includes a first bearing surface (200) that articulates with the first femoral bearing surface (162), a second bearing surface (202) that articulates with the first femoral bearing surface (164) and a third bearing surface (198) that articulates with the tibial bearing surface (196). The bearing member (156) further includes a posterior lip extension (284) that substantially inhibits the bearing member (156) from dislocating upon flexion of the knee prosthesis (146).

A nonmodular prosthesis having a retainer for a modular bearing and a nonmodular primary bearing directly molded to the base. A nonmodular tibial prosthesis is provided with a mechanical release member (12) mounted on a tibial base (10) in contact with the nonmodular primary bearing (26). The release member includes a wedge portion (16) with a superiorly posteriorly inclined face, and is designed to be driven anteriorly upon actuation so as to force the primary bearing off of the tibial base, after which the primary bearing and release member are removed and replaced with a modular bearing.

A trial system for an acetabular prosthesis (10) is described. The acetabular prosthesis (10) is generally for implantation in an acetabulum (302) and surrounding pelvis (300). The acetabular prosthesis (10) includes an acetabular cup (12) having a substantially concave inner surface (16) and a substantially convex outer surface (18). The described acetabular prosthesis (10) is especially useful in revision hip implant procedures where significant bone tissue loss has occurred either in or around the acetabulum (302) and/or the pelvis (300). A trial shell (200) is provided to trial a range of motion of the hip joint before implanting a prosthetic shell (320) into the acetabular prosthesis (10).

A work hardened articulating surface for anatomical prosthetics. Articulating surfaces of prosthetics are work hardened to decrease the wear and increase the longevity of the prosthetic after implantation. The articulating surface can be hardened in any number of ways, for example the articulating surface can be formed from stock material that has been work hardened throughout or the articulating surface can be work hardened prior to final finishing of the prosthetic component. Regardless, the hardened articulating surface reduces wear after implantation while eliminating the need for a separate bearing component.

A constraining modular hip prosthetic to reduce the possibility of a dislocation of the hip prosthetic after implantation. The constraining liner (14) is provided in a preassembled position including an opening smaller than the internal diameter of the liner. The modular head portion includes a equator diameter (28) which is small enough to be received into the liner. It includes a second dimension which is greater than the opening to constrain the modular head within the liner. A kit is also provided for trialing the modular hip prosthetic to provide and determine a customized and personal fit of the hip prosthetic for each patient.

An articulating bearing knee including a guide post that in a first orientation allows substantially free varus and valgus distraction, but in another constrains the same. The guide post in includes a superior portion that engages a femoral component of a knee prosthesis in a rotated orientation. When the guide post engages the femoral component it may not distract as if it was not engaged. This allows for a control and constraint of such distraction when the knee is rotated and in a weaker position.

The present invention provides a hip prosthesis (38) with a modular or integral trochanteric flange (56). The prosthesis (38) includes a stem (42) for insertion within a reamed out femoral canal and a head (50) for articulation within an acetabulum of a hip bone, the head (50) and stem (42) being connected by a neck (44). The neck (44) contains an attachment region (54) for securing the trochanteric flange (56) to the prosthesis (38). The trochanteric flange (56) is selected from a variety of trochanteric flanges differing in length. The specific trochanteric flange (56) selected depends upon the amount of hip joint soft tissue resected from a particular patient. If a large amount of soft tissue is resected an elongated trochanteric flange (56) will be selected. However, if little or no soft tissue is resected, a shorter trochanteric flange (56) will be selected. The ability to customize the size of the trochanteric flange (56) allows the physician to better recreate the natural relationship between the greater trochanter and the soft tissue of the hip joint, thus allowing the soft tissue to be secured to the trochanteric flange (56) without having to stretch the soft tissue or otherwise cause undesirable tension within the soft tissue.
