The present invention is directed to interbody spinal fusion implants (100) having a structural configuration that provides for the maintaining and creating of the normal anatomic angular relationship of two adjacent vertebrae (V2, V3) of the spine (S) to maintain and create spinal lordosis. The spinal fusion implants (100) of the present invention are sized to fit within the disc space created by the removal of disc material between two adjacent vertebrae (V2, V3) and conform wholly or in part to the disc space created. The spinal fusion implants (100) of the present invention have upper (112) and lower (114) surfaces that form a support structure for bearing against the end plates of the adjacent vertebrae (V2, V3). The upper (112) and lower (114) surfaces are disposed in converging angular relationship to each other such that the implants (100) of the present invention have an overall wedged shape in an elevation side view.

Apparatus and a method of inserting spinal implants is disclosed in which an intervertebral space is first distracted, a hollow sleeve having teeth at one end is then driven into the vertebrae adjacent that disc space. A drill (240) is then passed through the hollow sleeve (140) removing disc and bone in preparation for receiving the spinal implant which is then inserted through the sleeve (140). Apparatus and method of inserting spinal implants is disclosed in which an intervertebral space is first distracted to restore the normal angular relationship of the vertebrae adjacent to that disc space. An extended outer sleeve (140) having extended portions capable of maintaining the vertebrae distracted in their normal angular relationship is then driven into the vertebrae adjacent that disc space. A drill (240) is then passed through the hollow sleeve (140) removing disc and bone in preparation for receiving the spinal implant which is then inserted through the sleeve.

The spinal fusion implants (220, 320 and 420) of the present invention may be relatively solid or hollow and may have surface roughenings to promote bone ingrowth and stability. The spinal fusion implants (220, 320 and 420) of the present invention may have wells (260) extending into the material of the implant from the surface for the purpose of holding fusion promoting materials and to provide for areas of bone ingrowth fixation. A variety of surface irregularities may be employed to increase implant stability and implant surface area, and/or for the purpose of advancing the spinal fusion implant into the fusion site.

This invention is an oversized spinal implant (100) for trans-lateral insertion into the disc space between two vertebrae, a length that is greater than one half of the transverse width of the vertebrae, and is greater than the depth of the vertebrae. The trans-lateral implant of the present invention has a height that is greater than the height of the disc space between two adjacent vertebrae so as to engage both of the vertebrae. The width of the implant need be only slightly less than the depth of the vertebrae themselves. The trans-lateral spinal fusion implant of the present invention has more surface area of contact, and thus permits greater stability so as to withstand torque, and in the case of a threaded implant, increases the depth in which any threads are able to penetrate the vertebrae.

An improved method and instrumentation for performing spinal surgery, including discectomy, interbody fusion and rigid internal fixation of the spine, from the lateral aspect of the spine is disclosed. The surgical procedure can be performed through a very small incision. The instrumentation of the present invention, all of which is inserted from a lateral position into the spine in the preferred embodiment, comprises a guide pin (30), a distractor (100), an extended outer sleeve (140), an inner sleeve (242), an adjustable drill (250), and an implant driver. The distractor (100) of the present invention is driven into the disc for spacing apart and realigning the adjacent vertebrae. It further functions as an alignment rod for inserting the extended outer sleeve which is a hollow tubular member capable of maintaining said spacing and alignment of two adjacent vertebrae, and defines a protected space through which subsequent instruments which may be included.
