The present invention involves systems (20) and related methods for performing dynamic pedicle integrity assessments involving the use of neurophysiology.
摘要:
The present invention involves a system and methods for assessing the state of the neuromuscular pathway to ensure further nerve tests aimed at detecting at least one of a breach in a pedicle wall, nerve proximity, nerve direction, and nerve pathology, are not conducted when neuromuscular blockade levels may decrease the reliability of the results.

The present invention involves systems (20) and related methods for performing percutaneous pedicle integrity assessments involving the use of neurophysiology. The fundamental method steps for performing the percutaneous pedicle integrity assessments include (a) percutaneously introducing an insulation member to a pedicle target site; (b) establishing electrical communication between a stimulation element and an interior of a pedicle screw pilot hole; (c) applying a stimulation signal to the stimulation element; and (d) monitoring to assess whether nerves adjacent the pedicle are innervating as a result of the step of applying the stimulation signal to the stimulation element.
摘要:
The present invention includes a system and methods aimed at surgery, and more particularly to a system and methods for monitoring nervous tissue to assess the integrity of a pedicle wall during or after pilot hole formation and before, during, or after screw implantation, particularly in the thoracic spine. The system also performs other neurophysiologic assessments including, but not necessarily limited to, neuromuscular pathway status, nerve proximity detection, nerve pathology monitoring, and spinal cord health monitoring.

The present invention involves systems and related methods for performing surgical procedures and assessments, including the use of neurophysiology-based monitoring to: (a) determine nerve proximity and nerve direction to surgical instruments employed in accessing a surgical target site; (b) assess the pathology (health or status) of a nerve or nerve root before, during, or after a surgical procedure; and/or (c) assess pedicle integrity before, during or after pedicle screw placement, all in an automated, easy to use, and easy to interpret fashion so as to provide a surgeon-driven system.

A system (10) for performing somatosensory evoked potential monitoring during surgery comprises a stimulator (22) to deliver an electrical stimulation signal to a peripheral nerve, a sensor (24) to detect somatosensory responses evoked by the stimulation signal and a control unit (12) communicating with the stimulator and the sensor. The control unit is configured to direct transmission of the stimulation signal and receive the response data from the sensor. In addition, the control unit is configured to optimize signal parameters of the stimulation signal based on the response data, assess spinal cord health by identifying a relationship between the response to a first stimulation and to a second stimulation, the relationship being at least one of a change in latency and a change in amplitude between the first and second responses. The control unit is also configured to communicate the assessment to a user.
