Methods and systems of applying treatment to a subject experiencing cardiac distress. In one example, there is provided a defibrillation electrode. The defibrillation electrode includes a non-volatile memory element and a circuit configured to store an indication of a level of energy delivered to a subject during the application of a defibrillation shock to the subject in the non-volatile memory element.

A method for managing care of a person receiving emergency cardiac is disclosed and involves monitoring, with an external defibrillator, multiple parameters of the person receiving emergency cardiac assistance; determining from at least one of the parameters, an indication of trans-thoracic impedance of the person receiving emergency cardiac care; determining, from at least one of the parameters corresponding to an electrocardiogram of the person receiving emergency cardiac assistance, an initial indication of likely shock effectiveness; determining, as a function of at least the indication of trans-thoracic impedance and the initial indication of likely shock effectiveness, an indication of whether a shock provided to the person receiving emergency medical assistance will be effective; and affecting control of the defibrillator by a caregiver as a result of determining the indication of whether a shock will be effective.

An atrial event and a ventricular event can be received, and an atrioventricular (AV) delay can be provided using information about the atrial and ventricular events. The AV delay can be increased after a first condition is satisfied to allow a heart to regain intrinsic control of ventricular activation, and changed after a second condition is satisfied to allow the heart to remain in intrinsic control of ventricular activation.

Techniques are provided for use with an implantable medical device for detecting breaches in lead insulation or other lead failures. In one example, bipolar impedance is measured along single-lead vectors (i.e. intra-lead vectors) of a right atrial (RA) lead and a right ventricular (RV) leads. Impedance is also measured along various cross-lead vectors (i.e. inter-lead vectors) between electrodes of the two leads. A derived impedance value is then determined from a combination of the measured impedance values, wherein the derived impedance is sensitive to a shunt impedance arising from a breach within the RV lead. A lead breach is then detected relatively early based on the derived impedance by detecting a significant deviation in derived impedance over time. Unipolar impedance measurements are used to confirm the breach.

A transthoracic defibrillator for external defibrillation comprises at least three electrodes configured to be attached to the thorax of a patient to establish at least two electrical paths across the thoracic cavity and through the heart of the patient. In addition, a defibrillator circuit contained in a defibrillator housing has the capability to deliver a different defibrillation waveform across each of the at least two electrical paths.

Methods of nerve signal differentiation, methods of delivering therapy using such nerve signal differentiation, and to systems and devices for performing such methods. Nerve signal differentiation may include locating two electrodes proximate nerve tissue and differentiating between efferent and afferent components of nerve signals monitored using the two electrodes.

A wearable defibrillation system can establish a local comlink with a mobile communication device, such as a smartphone, tablet-type computer and the like. The mobile communication device can in turn establish a remote comlink with other devices in a network such as the internet. Accordingly, communication tasks relating to the wearable defibrillation system can be performed via the local and the remote comlinks, with or without the participation of the patient, who is wearing the system. The wearer can thus use the familiar interface of a mobile communication device for interacting with his defibrillator system. Moreover, he can do so while keeping on his regular clothes, which could conceal completely the wearable defibrillator system. The patient can thus preserve his dignity and privacy.

According to some embodiments, a wearable medical device capable of treating a patient presenting with syncope is provided. The wearable medical device includes a memory storing event profile information, a battery, at least one treatment electrode coupled to the battery, at least one processor coupled to the memory and the at least one treatment electrode, and an event manager executed by the at least one processor. The event manager is configured to detect an event associated with syncope; store, in the memory, data descriptive of the event in association with an indication that the data includes data descriptive of a syncopal event; and address the to event.

A medical device includes an energy storage device, a plurality of electrodes, a memory, a switching circuit, and a processing module. The energy storage device stores electrical energy for delivery of defibrillation therapy to a heart. The memory stores N therapy configurations, each of the N therapy configurations defining which of the plurality of electrodes are used to deliver defibrillation therapy and further defining a waveform to be applied during delivery of defibrillation therapy. The switching circuit is configured to connect the plurality of electrodes to the energy storage device. The processing module is configured to control the switching circuit to deliver defibrillation therapy according to a first therapy configuration, detect a fault during delivery of the defibrillation therapy according to the first therapy configuration, and select a second therapy configuration based on when the fault was detected during delivery of the defibrillation therapy according to the first therapy configuration.

Devices configured for and methods for inducing fibrillation in a patient using a controlled current AC signal applied via electrodes of an implanted ICD. In some embodiments, the AC signal is applied as a series of alternating constant current pulses. Some embodiments make use of a specialized H-bridge circuit for applying the AC signal. A low-side current controlling portion of an ICD's circuitry may make up part of the specialized H-bridge circuit. Further embodiments include devices embodying these methods.
