    摘要: An endoscopic surgical apparatus is provided for placing lateral lines of surgical fasteners into body tissue. The apparatus includes a frame portion, an elongated portion extending from the frame portion, and an articulating fastener applying assembly associated with a distal end of the elongated portion. The fastener applying assembly includes a base portion, a staple cartridge housing, and an anvil member which has a forming surface thereon against which surgical fasteners are driven as they are ejected from the cartridge housing. A first mechanism is provided for effectuating the rotation of the fastener applying assembly about an axis defined by the body portion, a second mechanism is provided for effectuating the articulation of the fastener applying assembly, and a third mechanism is provided for independently rotating the cartridge housing and anvil member relative to a longitudinal axis defined by the base position to increase the range of operability of the apparatus.

    摘要: An instrument including a clamping jaw with an anvil defining an elongated slot and staple forming cups on opposing sides of the elongated slot. A housing rotatably mounts on the anvil and a pusher rotatably mounts within the housing. A handle actuates the clamping jaw from the open to the intermediate position by rotating the housing towards the clamping jaw. To move from the intermediate position to the closed position, the pusher rotates towards the anvil. A staple cartridge, disposed in the housing, includes a staple track for retaining two rows of staples on opposing sides of the elongated slot. Two staple drivers align with the distal most staples. The drivers slidably mount in the staple cartridge and couple to the pusher such that each staple driver forces a staple into the forming cups and, in turn, staples the body tissue as the pusher rotates from the intermediate to closed position. A knife rotatably mounts on the staple cartridge and couples to the pusher such that the knife passes into the elongated slot and, in turn, cuts the body tissue as the pusher rotates from the intermediate to closed position.

    摘要: A vascular suturing device is disclosed which includes an elongated tubular body defining opposed proximal and distal end portions and having a longitudinal axis extending therethrough, the body including an inner tubular member, an outer tubular member and a central tubular member disposed between the inner and outer tubular members. The inner tubular member and the central tubular member are mounted for movement relative to the outer tubular member about the longitudinal axis of the body for sequentially driving a pair of suture needles through the wall of a blood vessel to close an incision formed therein. A vascular dilator having a tapered distal tip portion is supported within the inner tubular member and is dimensioned to extend beyond the distal end of the inner tubular member for positioning the suturing device at the incision in the wall of the blood vessel. The dilator is configured to direct blood flow from the distal portion of the dilator to a location remote from the blood vessel for observation to confirm the location of the device.

    摘要: A disposable apparatus is disclosed for applying latchless surgical clips to body tissue in endoscopic surgical procedures. The latchless clips are resiliently biased to a closed configuration. The apparatus includes a frame which is adapted to be gripped by hand, and an endoscopic section connected to the handle and capable of storing surgical clips in preparation for clipping arteries or other body tissue. The apparatus includes means for advancing each clip sequentially, temporarily opening the clip and advancing the clip further to a pair of distal jaws where the clip is positioned around the body tissue and closes around the tissue. Also included are means for closing the jaws about the clip. When the jaws are closed, the clip advancing means is simultaneously positioned to advance the next clip. The present apparatus also makes it possible to partially close a clip without interfering with the sequential movement of the clip advancing mechanism.

    摘要: A detachable anvil rod for use with an apparatus for circular fastening of hollow organs is provided. An elongated rod member has a proximal end portion, a distal end portion and an irregular surface portion intermediate the proximal and distal end portions. The irregular surface portion includes a plurality of grooves which define a plurality of raised ridges. The ridges are adapted to engage grasping surfaces of a grasping tool so as to minimize potential of slippage, thus enhancing the maneuvering procedure of the anvil rod about the operative site. The anvil rod also includes a narrowly configured mounting portion which enables the rod to be applied to and mounted within the distal end of the stapling apparatus from a variety of angular positions. An apparatus which incorporates the improved detachable anvil rod is also described.

    摘要: An apparatus and method are disclosed for applying a surgical clip to an exterior wall of a blood vessel to at least partially close a hole formed therein. The apparatus includes a handle portion, an elongated body extending distally from the handle portion and dimensioned to extend through a hole in the wall of a blood vessel, and a collapsible locator associated with a distal end portion of the elongated body and mounted for movement between a collapsed position and an expanded deployed position. The locator is adapted to expand within an interior lumen of the blood vessel to maintain the distal end portion of the elongated body in a desired location with respect to the hole in blood vessel wall such that a surgical clip releasably supported adjacent the distal end portion of the elongated body can be applied to the exterior wall of the blood vessel to close the hole formed therein.

    摘要: A self contained gas powered endoscopic surgical apparatus is provided for placing lateral lines of surgical fasteners into body tissue. The apparatus includes a frame portion, an elongated portion extending from the frame portion, and an articulating fastener applying assembly associated with a distal end of the elongated portion.
