In accordance with the present invention, there is provided a lifting apparatus for pivoting a surgical retractor with respect to a patient it is being used on. The lifting apparatus has a bridge for engaging at least one arm of a surgical retractor. The bridge has a distal end, a proximal end and a longitudinal axis extending therebetween. The bridge further includes a distal coupling attached to the distal end of the bridge for releaseably attaching the bridge to the surgical retractor. The lifting apparatus further comprises an elevator. The elevator is for applying an upward force to the proximal end of the bridge, so that when the apparatus is attached to a surgical retractor, the elevator pivots the retractor upward about the distal coupling. The elevator includes a bridge coupling for attaching to the proximal end of the bridge. A resilient pad is releaseably attached to the base of the lifting assembly and is positioned between the base and the chest of the patient.

The adhesive fastener can be formed as a polymerized shank or shaft extending through the tissue, and can have one or more heads formed on the shank. The shank can be formed solely from the polymerizable adhesive, or can be formed from another material such as nitinol, titanium, stainless steel, suture, or a plastic. Shanks of other material can be combined with the adhesive. Alternately, an adhesive fastener can be formed extending distally from a distal end of the surgical device. And, an adhesive fastener can be formed around a luminal tissue or within luminal tissue.

In one version particularly suited to an open surgical procedure, the ampoules are affixed to a tape substrate that is unreeled from an internal storage reel and transported proximate to an ampoule gripping mechanism with an underlying tape substrate peeled away and internally stored as each ampoule is positioned for dispensing. Actuating an ampoule advancement handle positions the next ampoule and squeezing a movable handle toward a fixed handle effects dispensing. Another version has an elongate shaft that internally augers each ampoule in succession into a pair of spaced jaws that are pinched together by actuating rods to dispense adhesive.

A ring applicator for applying elastic rings to anatomical tissue during surgical procedures includes an inner member with a forceps movably disposed within a middle tubular member having a distal end configured to hold a plurality of elastic rings in an expanded state and a proximal end with a plurality of longitudinally spaced steps. The middle tubular member is movably disposed in an outer tubular member, and an adjustable stop mechanism is mounted on the proximal end of the outer tubular member to limit axial movement of the outer tubular member relative to the middle tubular member to select which rings are to be ejected. A distal handle is connected with the inner member via slots in the outer and middle tubular members, and a proximal handle is pivotally connected to the middle tubular member via a slot in the outer tubular member. A pusher has a distal end movable between a latched or locked position where the distal end of the pusher protrudes into apertures in the outer and middle tubular members to prevent pivotal movement of the proximal handle and an unlatched or unlocked position where the distal end of the pusher is disengaged from the aperture in the middle tubular member to permit pivotal movement of the proximal handle in order to move the outer tubular member axially relative to the middle tubular member to eject an elastic ring. Distal portions of the outer and middle tubular members are preferably transparent with scale markings formed thereon to allow the surgeon to accurately determine the length of the tissue disposed within the middle tubular member prior to ejecting a ring. A prerelease feature is also disclosed for unlocking the pusher to allow ejection of a ring when the forceps has not been completely withdrawn in to the middle tubular member.
