摘要: A system for performing an end-to-side vascular anastomosis, including an anastomosis device, an application instrument and methods for performing a vascular anastomosis. The system is applicable for performing an anastomosis between a vascular graft and the ascending aorta in coronary artery bypass surgery, particularly in port-access CABG surgery. A first aspect of the invention includes a vascular anastomosis staple. A first configuration has two parts: an anchor member, forming the attachment with the target vessel wall and a coupling member, forming the attachment with the bypass graft vessel. The anastonosis is completed by inserting the coupling member, with the graft vessel attached, into the anchor member. A second configuration combines the functions of the anchor member and the coupling member into a one-piece anastomosis staple. A second aspect of the invention includes an anastomotic fitting, having an inner flange over which the graft vessel is everted and an outer flange which contacts the exterior surface of the target vessel. A tailored amount of compression applied by the inner and outer flanges grips the target vessel wall and creates a leak-proof seal between the graft vessel and the target vessel. A third aspect of the invention has a flange to which the graft vessel attaches, by everting the graft vessel over the flange, and a plurality of staple-like members which attach the flange and the everted end of the graft vessel to the wall of the target vessel to form the anastomosis

摘要: An apparatus for closing wide skin defects may be used with two long interdermal needles configured for insertion underneath skin close to margins of a skin defect. The skin closing apparatus may include two U-shaped retaining members configured as a flange connecting two legs and forming a U-shaped profile. Each of the legs may include a sharp hook at the end of the leg adapted for piercing the skin and engaging one of the long interdermal needles. Each retaining member may include a pivoting platform that pivots up to 20.degree. from an initial position indicated by a detent mechanism. A contracting mechanism such as a screw may connect the retaining members and approximate the interdermal needles, while bringing the skin margins together until final closure of the skin defect results. The skin closing apparatus may also include a clutch mechanism configured to disengage the screw from a knob or handle, when a predetermined force is met or exceeded. A visual or auditory element may also be provided to indicate the amount of force applied to the skin. A retractor may be packaged with the skin closing apparatus and long interdermal needles in a presterilized kit. The retractor may exhibit atraumatic ends having curved edges. The retractor may also exhibit english and metric measuring scales to aid in placing the skin closing apparatus and long interdermal needles about the skin defect. The device may be configured as a single use device utilized with gamma sterilization. In the event the device is configured for multiple uses, any conventional sterilization method may be utilized.

摘要: A monopolar needle assembly comprises a disposable insulated needle having an uninsulated tip projecting from a needle stem and a sheath for covering the needle. The protective sheath is preferably plastic and injection molded. The needle is disposed within a protective sheath until the needle is inserted into the patient, thereby preventing contamination of the needle. As embodied herein, the exterior and interior surfaces of the protective sheath are tapered to conform essentially to the elongated configuration of the needle. The portion of the sheath proximate the needle stem includes flanges projecting from the exterior of the sheath to prevent the monopolar needle assembly of the present invention from rolling if it is dropped.
