An emboli capturing system captures emboli in blood flowing in the vasculature. The emboli capturing system includes a guidewire having a longitudinal axis and defining a lumen along at least a portion thereof. An expandable member is coupled to a distal portion of the guidewire and has an interior being in fluid communication with the lumen in the guidewire. The expandable member is configured to receive fluid through the lumen to expand radially outwardly relative to the guidewire and have fluid removed from the interior thereof to collapse radially inwardly relative to the guidewire. The expandable member, when expanded, has a spaced portion thereof spaced radially outwardly from the guidewire. An emboli capturing device is coupled to the expandable member and is configured to deploy radially outwardly relative to the guidewire upon expansion of the expandable member.

Apparatus and method are disclosed for facilitating balloon catheter exchange in the vascular system of a patient. The apparatus and method concern means for holding a guidewire longitudinally fixed with respect to a guiding catheter while a balloon catheter is moved over the guidewire. Specifically, the guidewire is held longitudinally fixed with respect to the guiding catheter by way of magnetic coupling. A first magnetic element constitutes a portion of the guide wire or is attached to the guide wire to move with it. A second magnetic element is located proximate the balloon catheter but is not fixed to the balloon catheter. When the guidewire is moved within the guiding catheter to a location at which the first magnetic element reaches its point of closest approach to the second magnetic element, magnetic force holds the guidewire against movement relative to the guiding catheter even while a balloon catheter is being moved over the guidewire.

In general, the methods include the steps of advancing a wire into the patient from a first location, through a neural foramen, and out of the patient from a second location; connecting a tissue modification device to the wire; positioning the tissue modification device through the neural foramen using the wire; modifying target tissue in the spine by moving the tissue modification device against the target tissue; and delivering an agent to modified target tissue, wherein the agent is configured to inhibit blood flow from the modified target tissue. In some embodiments, the step of modifying target tissue comprises removing target tissue located ventral to the superior articular process while avoiding non-target tissue located lateral to the superior articular process.

The wireguide passes through one access site while the catheter passes through another access site. The wireguide and catheter are connectable to each other inside the patient's body. Once connected, the catheter can pull the wireguide through the access site of the catheter. The wireguide set may be used to treat peripheral arterial disease occlusions in the lower leg where it is difficult or impossible to gain access through the occlusion from a femoral site.

An attachment mechanism and method for attaching or coupling a guide wire to a catheter for re-canalization or opening a passage through an occlusion in a blood vessel is provided. The attachment mechanism generates friction forces against the guide wire to couple the guide wire to a catheter, which design permits a greater force to be used on the guide wire during a re-canalization procedure. The invention also encompasses use of the attachment mechanism in conjunction with active catheters, which have vibration-generating means to oscillate the distal end of the catheter or a component in the distal end of the catheter, to add vibration motion, preferably axial vibration motion, to the increased force that may be applied to the guide wire for purposes of penetrating an occlusion. The methods of the invention relate to methods of attaching the guide wire to the catheter and methods of treating a vessel having a partial or total occlusion.

The guide catheter apparatus includes a substantially rigid shaft and a handle. The shaft has a proximal opening, a distal opening, and a lumen extending between the proximal opening and the distal opening. The handle has a structure to allow a position of the guide catheter to be controlled by some or all of three fingers of one hand of an operator of the handle.
