A system and method for determining on a continuous, real-time basis the proximity of the esophagus to an endocardial catheter during mapping, ablation or other endocardial catheter-based procedures, comprising an esophagus probe catheter and an endocardial catheter adapted for proximal signal transmission between each other. A signal processing unit is included to process and compare a characteristic of the proximity signal that is changes or attenuates with distance between the two catheters, such as impedance, amplitude and/or phase. Audio and/or optical outputs are provided to alert an operator when the distance between the catheters changes or is below a baseline measurement to avoid damage to the esophagus by the endocardial catheter. The system and method may include adaptations of the catheters with location sensor, and a mapping/navigational system for nonfluoroscopic location determination of the catheters.

The body (250, 350) may be cylindrical with the antenna (240, 340) along its axis. The antenna may be λ/2 in length and λ/2 in radius. The tip (270) of the antenna (240) may be rounded hemispherical with radius λ/2 to enhance forward transmission of radiation. The dielectric constant (ε) of the body (250, 350) is as high as possible to reduce its diameter at a desired operating frequency but may be matched to the surrounding tissue by another layer of dielectric material (380) with a value (ε) intermediate that of the core (360) of the body (350) and the tissue.

A transcutaneous electrode is disclosed having a sheet electrode of an electrically-conductive material with an electrically conductive layer affixed to a major portion of the lower surface thereof. A pad of electrically-conductive gel is applied to the lower surface of the sheet electrode over the electrically-conductive layer. An electrical conductor having an unsheathed end portion is secured to the upper surface of the sheet electrode by an electrically-conductive adhesive. A high dielectric cover overlays the end out portion of the electrical conductor, and is secured to the sheet electrode by the conductive adhesive. A method for making the electrode is also disclosed.

A medical electrode for obtaining biopotentials from the skin of a subject or electrically stimulating the subject's skin and deeper tissue layers. The electrode has a carrier base member from which project a plurality of spikes arranged in an array on one surface of the base member. The spikes are sufficiently long to penetrate through the stratum corneum into the stratum germinativum of the subject's skin. The spikes may be formed by a deep reactive ion etching process on a silicon wafer forming the base member. A fluid container may be formed on another surface of the skin for providing a drug to the surface of the skin through holes in the base member. The action of the spikes on the skin enhances administration of the drug.

Such a system allows accurate control of timing and/or amount of release.
