    摘要: An apparatus for sealing a passage through tissue includes a bioabsorbable body disposed on a distal end of a handle device. The plug member includes a helical thread on its outer surface and the plug member and handle device include cooperating lumens communicating with a distal port. A sealing member is disposed within the plug member lumen. An obturator is inserted through the lumens until its distal tip extends beyond the plug member, the distal tip including a bleed back port. The plug member is threaded into a passage towards a blood vessel until the bleed back indicator identifies the location of the vessel with respect to the plug member. The plug member is released from the handle device, the handle device and obturator are withdrawn, and the sealing member seals the lumen. The plug member is left within the passage to seal the passage until the tissue heals.

    摘要: A ligation device includes a cannula having first and second extendable members and a Doppler wand. The Doppler wand permits an anatomical vessel, such as a blood vessel, to be located in close proximity to the distal end of the cannula. The extendable members are extendable on two opposite sides of the vessel. A length of ligation material, such as suture material, extends through the first extendable member, and is attached to a detachable element mounted to the distalmost end of the extendable member. The second extendable member includes a snare. The first extendable member includes curved portions which, when the member is extended distally, extend the detachable element into position for the snare so that the snare can grab the loop. Once ensnared, the detachable element is released from the first extendable member, which pulls the ligation material around the vessel. The ligation device does not require prior visualization or dissection of the area around the vessel.

    摘要: An attachment assembly and repair graft are disclosed for securing a graft to repair a vessel having an aneurysm therein. The attachment assembly comprises an attachment cuff such that the graft is not dimensionally dependent upon the size of the vessel. A visualization apparatus is also disclosed for real time direct viewing of an interior of a vessel. A penetration apparatus is disclosed for use in forming treatment specific holes in a potentially calcified vessel wall which facilitates thereafter the securing of the graft and attachment assembly to the vessel wall. An introducer sheath device is also disclosed that comprises a sealing assembly for preventing the loss of blood from the vessel during the insertion and subsequent removal of surgical components during the surgical procedure.

    摘要: A constriction device that constricts body tissue is configured to be safely sutured to constricted body tissue. The device includes a generally cylindrical elastic sleeve including opposed opened ends and having a wall of substantially uniform thickness that receives body tissue therein to be constricted. The sleeve includes a plurality of suture holes to receive a suture to maintain the sleeve the tissue constricted by the sleeve. The sleeve includes a suture hole reinforcement structure, as for example an increased wall thickness, about each suture hole to permit the device to be sutured to constricted body tissue without damaging the device.

    摘要: A device for incising a stenosis in the vasculature of a patient includes a plurality of blades mounted on a resilient base member. The blades are arranged in a pattern to allow for relative movement between adjacent blades. Specifically, at least one end portion of each blade is juxtaposed with an end portion of the next closest blade. The base member, in turn, is mounted on the external surface of an inflatable angioplasty balloon. When the balloon is inserted into the vasculature of a patient, positioned across a stenosis and subsequently inflated, the individually moveable blades form an effective cutting edge that conforms to the surface of the stenosis to effectively allow the stenosis to be incised to a substantially uniform depth.

    摘要: An aorta cross clamp assembly includes a clamp having movable jaws and a removable actuator having movable handles. Initially, the actuator is connected to the clamp with the jaws in an open position. When the actuator handles are closed, the jaws also will be closed. The clamp is provided with a toothed retainer that prevents the jaws from being opened accidently. The actuator can be detached from the clamp and removed from the operative site, leaving the locked clamp in place. When it is desired to remove the clamp, the actuator can be reattached to the clamp and used to disengage the toothed retainer. The actuator then can be used to spread the jaws in order to permit the clamp to be removed. The clamp and the actuator are provided in two forms. In one form, the toothed retainer is engaged and disengaged by movement toward and away from the jaws (“horizontal” movement). In the other form, the toothed retainer is engaged and disengaged by movement generally perpendicular to the jaws (“vertical” movement). Another embodiment employs a clamp having movable jaws and a slender, flexible, actuator that is not intended to be removed during use. The jaws are actuated by axial movement of a screw that is connected to the end of a cable included as part of the actuator. The screw passes through a slotted nut that permits non-rotational axial movement of the screw in one direction to close the jaws, but which requires that the screw be rotated in order to move in the opposite direction to open the jaws.

    摘要: A surgical apparatus and method for occluding or encircling a body passageway, e.g. for haemostasis, is described. The apparatus comprises generally a delivery system for offering a temperature-dependent shape memory material clip (3′) (e.g. of Nitinol and suitably of generally U-shape in the open (a) condition) onto the body passageway, allowing body heat to warm the clip whereby the clip closes to a closed (b) condition in which it occludes or encircles the body passageway. The apparatus is then withdrawn from the operating zone. The apparatus includes drive and control functions for moving a train of clips (3′) within a shaft (2′) of the apparatus towards a distal end port (6′), and preferably a temperature control system whereby the clips within the housing are maintained at a first temperature, substantially below body temperature.
