An apparatus and method for providing pacing pulses to a second site when the pacing pulses are inhibited from being provided to a first site. A first cardiac signal and a second cardiac signal are sensed, where both the first and second cardiac signals include indications of cardiac events. A cardiac rate is determined from the cardiac events in one of the first cardiac signal or the second cardiac signal, and pacing pulses are provided to the first cardiac region in order to maintain the cardiac rate at at least a minimum rate value. A pace protection interval starts when a cardiac event is detected in the first cardiac signal. The pace protection interval inhibits delivery of pacing pulses to the first cardiac region. When pacing pulses to the first cardiac region are inhibited because of the pace protection interval, pacing pulses are provided to the second cardiac region at a safety interval timed from the inhibited pacing pulse to the first cardiac region.

Methods for failure recovery in a cardiac rhythm management system and apparatus capable of carrying out the methods. The methods include applying a first pacing therapy using one or more leads. The methods further include detecting a failure condition on one or more of the leads, wherein the failure condition prohibits or frustrates application of the first pacing therapy. The methods still further include applying a second pacing therapy using one or more of the leads subsequent to detecting the failure condition. The second pacing therapy is preferably chosen such that the detected failure does not interfere with the second pacing therapy. The second pacing therapy may be applied for only one cardiac cycle. The second pacing therapy may further be applied continuously until the failure condition is resolved, or it may be latched such that physician intervention is required to resume the first pacing therapy.

A cardiac rhythm management system includes techniques for computing an indicated pacing interval, AV delay, or other timing interval. In one embodiment, a variable indicated pacing interval is computed based at least in part on an underlying intrinsic heart rate. The indicated pacing interval is used to time the delivery of biventricular coordination therapy even when ventricular heart rates are irregular, such as in the presence of atrial fibrillation. In another embodiment, a variable filter indicated AV interval is computed based at least in part on an underlying intrinsic AV interval. The indicated AV interval is used to time the delivery of atrial tracking biventricular coordination therapy when atrial heart rhythms are not arrhythmic. Other indicated timing intervals may be similarly determined. The indicated pacing interval, AV delay, or other timing interval can also be used in combination with a sensor indicated rate indicator.

Using information produced by an implantable respiration sensor circuit and an implantable physiological sensor circuit, such as at least one of a physical activity sensor circuit or a posture sensor circuit, an implantable or external processor circuit may detect a respiration disturbance and an associated subsequent arousal from stable state occurrence and thereafter evaluate over time arousal from stable state occurrences to provide the indication of present or impending worsening heart failure. In one example, information about a fluid level within a subject is used in determining the indication of worsening heart failure. In various examples, a regimen is initiated or adjusted in response to the indication of present or impending worsening heart failure.

Also disclosed is a method for assessing a cardiovascular pressure of a patient with an implanted medical device that includes detecting one or more characteristics of a cardiac cycle of the patient's heart; detecting the cardiovascular pressure of the patient; classifying the cardiovascular pressure based on one or more characteristics of the cardiac cycle of the patient's heart; and displaying the cardiovascular pressure along with the classification. Also disclosed is a system for collection and display of a cardiovascular pressure of a patient.

Devices, systems, and methods for sensing cardiac electrical activity and blood pressure within a blood vessel are disclosed. A vascular sensor includes a fixation element configured to secure the vascular sensor within a blood vessel, a sensor module housing coupled to the fixation element, a pressure sensor disposed within an interior portion of the housing and configured to sense blood pressure within the vessel, and a number of electrocardiogram sensing electrodes exposed to tissue and blood within the blood vessel and configured to sense cardiac electrical data within the blood vessel. A controller module processes the sensed cardiac electrical data and blood pressure data and stores the data within a memory unit and/or transmits the data to a communicating device in wireless communication with the vascular sensor.
