摘要: The present invention is an integrated and automated irrigation and aspiration system for use in an endoscopic imaging system. The system provides for the automated cleaning of poorly prepared patients during a colonoscopy procedure as well as automated cleaning of an imaging system of an endoscope. The invention analyzes images obtained from an image sensor to detect the presence of an obstructed field of view, whereupon a wash routine is initiated to remove the obstruction. The wash routine may be adjusted in accordance with environmental conditions within the patient that are sensed by one or more sensors within the endoscope. In another embodiment, insufflation is automatically controlled to inflate a patient's colon as a function of one or more sensor readings obtained from one or more environmental sensor(s) on the endoscope.

摘要: An integrated catheter assembly for enabling diverse endoscopic in situ therapies. The assembly includes a catheter with an irrigation fluid lumen, a distal electrode tip portion that acts as a hemostat, and a cutting wire for making incisions in or ablating tissue. A cutting wire hub provides a sealing entrance for a cutting wire. An operator enables the physician to displace the cutting wire between extended and retracted positions. The cutting wire and electrode are electrically isolated. In addition, the catheter assembly may include a planar tip which, when extended outside the distal end of the assembly, assumes a substantially flat unbiased configuration for use. The apparatus of the present invention allows the physician to make incisions in or ablate tissue using electrosurgery, irrigate tissue, and cauterize or coagulate tissue without having to remove the apparatus from the working channel of the endoscope.

摘要: The invention in certain aspects relates to a surgical fastener for fastening tissue segments having tissue surfaces. The fastener includes a first fastener member having a base and a piercing element connected to the base for piercing the tissue segments to be fastened, a second fastener member having an opening for receiving and retaining the piercing element of the first fastener member such that the tissue segments to be fastened are retained between the first and second fastening members, and means for promoting adhesion between the tissue surfaces. The invention also relates to related methods and devices for promoting adhesion of tissue segments and preventing fastener migration, especially in an endoscopic procedure for the treatment of GERD.

摘要: The invention features a catheter having a catheter body of extended length for insertion within a body of a living being. The catheter also includes an ultrasound imaging device disposed within a distal portion of the catheter body. The ultrasound imaging device generates electrical signals used by an ultrasound imaging system to display a real-time image of tissue surrounding the distal portion of the catheter. The catheter further includes a cutting element, e.g., an electrode wire or a laser fiber, having a proximal portion disposed within the catheter body. The catheter also includes a mechanism connected to the catheter body that causes the distal portion of the cutting element to extend from the catheter body. The distal portion of the cutting element generates energy in tissue with which it is brought in proximity to form a cut in the tissue. The ultrasound imaging device is positioned relative to the cutting element such that the real-time image produced by the ultrasound imaging system can include the cutting element in relation to the tissue.
