    摘要: A laparoscopic bipolar electrosurgical instrument can apply a large closure force between its jaws without damaging the small yoke assembly. The instrument comprises: a first jaw having a first flange with a first slot, and a second jaw having a second flange with a second slot, wherein the first and second jaws are located at a distal end of the instrument and comprise an electrically conductive material for conducting bipolar electrosurgical current therebetween; a yoke attached to a pushrod and positioned to electrically insulate the first flange from the second flange, the yoke having a first side facing the first flange and a second side facing the second flange, the yoke further comprising a first shoulder and a second shoulder, a first pin located on the first side and movably engaged with the first slot; a second pin located on the second side and movably engaged with the second slot; the first slot and the second slot shaped such that an angle, subtended by the first and second jaws, decreases with distal motion of the pushrod, and first and second cul-de-sacs positioned respectively in the first and second slots to relieve shear stresses on the first and second pins approximately when the first and second shoulders respectively engage the first and second flanges to provide a closure force between the first and second jaws.

    摘要: An apparatus for thermal treatment of a tissue mass having an elongated housing having a proximal and a distal end and defining a longitudinal axis, and further including at least two electrodes supported at its distal end and mounted for rotation to facilitate entry and passage through a tissue mass. A drive shaft may be disposed within the elongated housing and operatively engageable with the electrodes for causing their rotational movement. The apparatus may include an electrode gear and a drive shaft gear in cooperative engagement whereby rotation of the drive shaft causes corresponding rotation of the electrodes. Preferably, a motor is operatively connected to the drive shaft causing its rotational movement. Desirably, the distal end of the electrodes are threaded. A method for thermally treating a tissue mass is disclosed.

    摘要: A surgical system and associated method for sealing the passageway of a fluid-carrying vessel with a diameter up to 5 millimeters comprises an electrosurgical generator capable of delivering electrosurgical power, a surgical instrument electrically connected to the electrosurgical generator and adapted to transfer electrosurgical power from the electrosurgical generator to a pair of end effectors disposed at a distal end of the surgical instrument. The system also includes a power control circuit for controlling the delivery of radio frequency energy to the vessel through the end effectors, wherein the delivery of the radio frequency energy to the vessel comprises, raising the output current to a range below 1.75 Amperes RMS and the output voltage to a range below 135 Volts RMS. Radio frequency energy is applied to the vessel for a period of time while the power is held approximately constant. The flow of radio frequency energy is terminated when the impedance of the vessel being sealed reaches a predetermined level.

    摘要: Apparatus and methods are provided for ablating body tissue using radio frequency (RF) energy. A catheter having a lumen for delivering fluid can be inserted into a patient's body. A porous member, which may be expandable, is attached to the distal portion of the catheter. The porous member defines an interior region in communication with the lumen, such that the interior region is capable of receiving electrolyte fluid delivered from the proximal portion of the catheter. An RF electrode is disposed in the interior region and is configured for coupling to a source of RF energy, whereby RF energy may be transferred from the electrode to selected tissue areas in a patient's body via electrolyte fluid delivered through the lumen and into the interior region of the porous member.

    摘要: An electrophysiology energy treatment device for treating tissues within a patient's body with electrical energy comprises an elongate catheter tube having a distal end insertable into the patient's body. An electrode is within the catheter tube adjacent the distal end and locatable within the patient's body, and an electrolyte fluid flows within the catheter tube for electrically connecting the electrode to the tissues to be treated within the patient's body.
