摘要: Systems, devices, and methods are provided for securing soft tissue to bone, for securing one or more objects using a surgical filament, and for drawing two or more tissues together so they can be secured in a desired location. One exemplary embodiment of a surgical repair method includes selecting a surgical repair construct that includes a snare linkage having a collapsible snare and a collapsible loop coupled to the snare linkage. Steps can be performed as part of the method such that the snare of the snare linkage is collapsed around a portion of the loop passing through the snare. The snare linkage and loop construct can be coupled to tissue and bone such that the snare and loop are both advanced distally towards bone to bring the tissue into proximity with the bone. Other exemplary systems, devices, and methods for use with soft tissue repair are also provided.

摘要: A surgical filament snare assembly including an anchor capable of being fixated in bone and having a filament engagement feature. A first filament has a noose with first and second noose limbs connected, preferably slidably, to the filament engagement feature. The first and second noose limbs emerge from the anchor as first and second free filament limbs that are capable of being passed through tissue to be repaired and then passable through the noose. The noose is capable of receiving the free filament limbs and strangulating them when tension is applied to at least one of the free filament limbs and the noose to enable incremental tensioning of the tissue after anchor fixation. Preferably, the snare assembly further includes a flexible sleeve joining at least some portion of the first and second free filament limbs to facilitate passing of the free filament limbs as a single unit.

摘要: Systems, devices, and methods are provided for securing soft tissue to bone. One exemplary embodiment of a surgical repair construct includes an anchor, a filament having a snare on one end and a collapsible loop on another end, and a suture having a plurality of stationary protrusions. The suture is configured to be coupled to detached tissue and have its ends passed through an opening in the snare. The snare can be collapsed around the suture so at least one of the protrusions is proximal of the collapsed snare. The anchor can be disposed in bone and the filament coupled thereto. Accordingly, collapsing the snare around the suture couples the tissue to bone, and applying tension to a tensioning limb of the filament can collapse the loop to incrementally tighten and secure the tissue to bone. Other exemplary systems, devices, and methods for use with tissue repair are also provided.

摘要: Various surgical fluid jet cutting instruments for selective bulk removal and precision sculpting of tissue are provided. In one exemplary embodiment, the instrument can include a fluid delivery tube having a nozzle for forming a high pressure fluid jet, and at least two evacuation tubes that are adapted to be selectively and removably coupled to the fluid delivery tube. Each evacuation tube can have an evacuation port or jet-receiving opening that is adapted to be positioned opposite to and spaced apart from the nozzle for receiving the high pressure fluid jet. Each evacuation tube can also be adapted for a specific use. For example, the instrument can include a first evacuation tube that is adapted to allow bulk removal of tissue, and a second evacuation tube that is adapted to allow precision sculpting of tissue.

摘要: Various methods and devices are provided for cutting tissue using a high pressure fluid jet. In one exemplary embodiment, a high pressure fluid jet delivery device is provided having a nozzle adapted to direct fluid to a tissue site. A heating element is disposed on the nozzle and is adapted to heat fluid surrounding a high pressure fluid jet flowing from the nozzle to form at least one vapor bubble. The vapor bubble can reduce the interaction between the high pressure fluid jet and the fluid surrounding the high pressure fluid jet.

摘要: Systems and methods are provided for passively dissipating a rotational speed of a fluid-driven rotatable shaft. In particular, a dissipation member is provided for dissipating the rotational speed of a shaft having an end effector formed thereon. As the rotational speed of the shaft increases, the dissipation member will apply a counter-torque to the shaft, thereby limiting the rotational speed of the shaft. In an exemplary embodiment, the counter-torque has a non-linear dependence on the rotational speed of the shaft, such that the counter-torque increases at a rate greater than a rate of increase in the rotational speed. Thus, when the end effector of the rotatable shaft is placed in contact with tissue and the operational speed of the shaft is reduced, the dissipation member will apply a minimal or low counter-torque to the rotatable shaft. When the end effector of the rotatable shaft is removed from the tissue and the operational speed of the shaft increases, the counter-torque applied by the dissipation member will increase thereby limiting the operational speed of the shaft.

摘要: A high pressure fluid jet system is provided, which is useful for cutting hard material during a surgical procedure. The cutting of hard material is more efficient as the system delivers abrasive solid particles with the high pressure fluid. In an exemplary embodiment, abrasive solid particles can be mixed with a pressurized stream of fluid prior to delivery of the fluid to a nozzle of an application tool. Alternatively, the abrasive solid particles can be entrained in the pressurized fluid stream or the pressurized fluid stream can erode a solid or suspension form of the abrasive particles.
