    摘要: An endotracheal tube is disclosed that allows a patient to speak while intubated. The endotracheal tube includes a balloon for seating against the trachea and a collapsible portion proximal to the balloon that aligns with the vocal cords. The collapsible portion may be held open by an inner tube which translates within the endotracheal tube. Suction and balloon inflation conduits span the collapsible portion so that suction and balloon inflation may be performed while the patient is speaking. A small distal tube that bypasses the balloon provides some airflow while the patient speaks.

    摘要: Surgical apparatus and method includes a cannula that houses an endoscope and supports a dilating element near a distal end of the cannula. The dilating element has a dimension which is greater than the diameter of the cannula for enlarging a surgical cavity in tissue as the cannula is advanced through tissue at a surgical site to provide working space adjacent a target vessel within which surgical instruments may be conveniently manipulated. The dilating element of oval sided shape permits surrounding tissue to be pushed away or otherwise displaced away from the target vessel atraumatically. A locking mechanism is disposed on the cannula, which accepts a succession of mating dilating elements of progressively larger dimensions for successive insertion and enlargement of a surgical cavity as required. In one embodiment, the dilating element is made of rigid plastic, and in another embodiment, the dilating element is made of resilient material that may be confined within a retractable sheath which, in the extended position, encases and compresses the dilating element to a smaller dimension and which, in a retracted position, allows the dilating element to resiliently expand and enlarge the surgical cavity.

    摘要: Method and apparatus for dissecting tissue along a vessel include a cannula having a tubular body and at least one lumen extending the length of the body to receive an endoscope in the lumen with a viewing end disposed near the distal end of the body that is covered by a transparent, tissue-separating tapered tip. Endoscopic viewing through the tip is enhanced by tapering the inner walls thereof to a cusp adjacent the distal end of the tip in order to reduce visual distortion. Alternatively, a removable or deflectable tip exposes a dissection probe that is manipulatable along a vessel and past lateral branching vessels at the distal end of the cannula as visualized by the endoscope through the transparent tip. Tissue overlaying an artery, for example, the superior epigastric artery, is bluntly dissected to establish an elongated working cavity of bluntly-dissected tissue. Lateral arterial branches are doubly occluded and then severed to isolate the artery, a section of which is transected and anastomosed to the coronary artery to revascularize the coronary artery.

    摘要: A system and method for closing a fascial opening is disclosed. The system may include a strap that may be locked into each of a pair of anchors placed anterior to a muscle and on opposite sides of a fascial opening, one or more delivery tubes for delivering the anchors, and a needle for pulling the strap into each anchor. The method may include the steps of placing a dilating port through a skin incision, inserting an anchor through the dilating port, placing a strap into the body cavity, whereupon the strap may be pulled through the anchor. A second anchor may be placed on the contralateral side of the defect so that the strap may be pulled through the second anchor and cinched tightly closing the defect. The anchors may have a locking aperture that engages with the strap.

    摘要: Apparatus and methods for using the apparatus are disclosed for cardiac restraint. More specifically, the apparatus and methods are directed to accessing the pericardium, accessing the heart within the pericardium, and restraining the heart by at least partially enclosing the heart with the apparatus. An embodiment of a cardiac restraint apparatus according to the present invention comprises a jacket, the jacket having a rim which defines an opening for receiving a heart. The apparatus also comprises a knot pusher that has a hollow elongate body, as well as a strand that extends around the rim of the jacket and is tied into a slipknot. The strand is positioned such that at least one end portion of the strand extends through the knot pusher such that a distal end of the knot pusher can be moved into engagement with the slipknot, whereby pulling the end portion of the strand away from the heart while pushing the knot pusher against the slipknot and reducing the diameter of the opening defined by the rim. In addition, the apparatus comprises one or more guide elements that are attached to the jacket.
