An implantable multi-lumen, multi-conductor lead for use with an implantable medical device. The completed lead is an assembly of a polymeric lead body with several coiled conductors inserted into the lead body. The lead body includes several lumens having a generally rounded-corner triangular or "pie-shaped" cross-section. The conductors do not fill the lumens, and only contact the inner walls of the lumens at discrete points, rather than for extended sections of or all of the circumference of the conductors. Preferably the coils are sized so that they fit loosely within the lumens.

An implantable lead having a plastic stylet guide therein. A substantially cylindrical channel is provided in a flexible lead body made of silicone rubber, polyurethane, or the like. The cylindrical channel defines a lumen containing a stylet guide into which a stylet can be inserted during an implantation procedure. Texture on the inner wall of the stylet guide minimizes friction between the stylet and the stylet guide, facilitating easy insertion and removal of the stylet. One or more electrical conductors may also be provided.

A steerable stylet and manipulative handle assembly especially adapted for guiding the introduction of an endocardial pacing or cardioversion/defibrillation lead or a catheter to a desired location in a patient's cardiovascular system. The steerable stylet or catheter designed to be introduced into the lumen of a lead comprises an outer, elongated tubular member of a length sufficient to extend through the length of the lead or catheter from its proximal opening to the distal end thereof, a traction element or pull wire extending generally through the length of the tubular member and within its lumen except for a predetermined distance in the distal portion thereof, and a manipulative handle coupled to the proximal ends of the tubular member and wire which may be employed with one hand to both rotate the tubular member and impart a continuously variable curve in the distal portion thereof. The distal portion of the tubular member is especially configured to be either attached to or cooperate with the distal end of the pull wire so that proximally-directed traction applied by the handle to the wire causes the distal portion of the tubular member to bend. The distal portion of the tubular member is provided with openings to allow the wire to exit and re-enter the lumen of the tubular member in the distal portion so that traction applied to the wire bows the tubular member into a curve of desired radius. In use, the distal portion of the wire residing outside the lumen of the tubular member is constrained within the lumen of the lead or catheter. Traction applied by the manipulative handle to the wire is sufficient to cause the distal portion of the pacing lead to assume various angles of curvature from straight to approximately one hundred eighty degrees. The distal portion of the tubular member which bows in response to the applied traction is preferably cut out or flattened to avoid kinking. The manipulative handle may be employed with one hand to impart the desired degree of curvature while rotating the lead or catheter to direct its distal end through the venus system and into the desired site within a chamber of the heart or the coronary sinus. The ability to dynamically change the degree of curvature during advancement avoids the necessity of removing and recurving the stylet during the introduction of the lead or catheter.

A suture sleeve for facilitating the ligature of an implanted catheter or lead to a patient's vein or underlying tissue having a longitudinal sleeve throughbore for receiving the lead body extending through a sleeve body and a movable locking member manually actuable between a locked and an unlocked position. The throughbore of the movable locking member is aligned with throughbores of the sleeve body, and the movable locking member and transverse channel are shaped to bias the movable locking member to the unlocked position. The locking member is movable laterally in a locking direction in a transverse channel extending laterally across the sleeve body with respect to the sleeve throughbore to a locked position that diminishes the movable locking member throughbore thereby compressing the lead body and applying a relatively uniform pressure in a band extending around its circumference, thereby minimizing lead body shear stresses. The locked position is maintained by engagement of mating fixed locking detents on the sleeve body and movable locking detents on the movable locking member. The locking member is unlocked by applying release force, preferably through the jaws of a forceps, against fixed rails on one side of the sleeve body transverse channel and against movable rails on the side of the locking member extending through the transverse channel on the other side of the sleeve body to release the locking detents and allow the locking member to move laterally in the channel to the unlocked position. When the locking member is in both the locked and the unlocked positions, the locking member throughbore is in substantial coaxial alignment with the longitudinal throughbores in the sleeve body, which are elongated in the transverse channel direction, thereby minimizing bending of the lead body in either position.

The body is generally planar with pairs of holes therethrough for receiving the legs of U-shaped spring anchor members of surgical spring steel. Each pair of legs is connected at the upper ends thereof to a bight and have at their lower ends sharply pointed outwardly, and downwardly sloping curved prongs which are spring biased outwardly. In an upper armed position the prongs are retained in tensioned condition adjacent the lower surface of the body by means of one or two wedges inserted between the upper surface of the body and the bights. Activation is achieved by removal of the wedges which allows the legs to move downwardly in a spread apart, snap action movement so as to drive the prongs downwardly and outwardly into adjacent myocardial tissue.

A chronic myocardial pacing lead for the delivery of stimulation energy to and the sensing of electrical signals from the myocardium of a human heart. In one embodiment the lead is bipolar, having a sleeve electrode implanted into the myocardium and a pad electrode positioned on the epicardium. The lead includes a drug for delivery through the sleeve electrode to the myocardium. The disclosed lead is highly flexible to minimize tissue reaction, this permits the lead to offer relatively low pacing thresholds, high impedance, and excellent sensing in a configuration which is relatively easy to implant.
