    摘要: A vascular closure device used in closing openings in vessel walls while permitting post-operative flow through the vessel includes a housing, at least two tissue engaging members, and at least two jaw members. The tissue engaging members engage tissue portions on opposite sides of the vessel opening and move tissue to a desired, predetermined position. The jaw members are adapted, in an open position, for positioning about the tissue portions in the predetermined position, and in a closed position, to at least partially draw the tissue together to an at least partially approximated condition. An attachment member is associated with at least one of the jaw members and arranged to contact the tissue. In one embodiment, the attachment member is adapted for connection to an energy source whereby energy is transmitted through the attachment member to thermally fuse the tissue engaged by the jaw members to substantially close the opening.

    摘要: An arterial closure device for use following coronary catherization procedures to close arterial access openings through the arterial wall while permitting post operative flow through the artery includes a housing having proximal and distal ends, and defining a longitudinal axis, first and second arterial tissue everting members mounted adjacent the distal end of the housing and first and second jaw members mounted adjacent the first and second tissue engaging members. The first and second arterial tissue everting members are dimensioned for at least partial positioning within the arterial access opening in the arterial wall and are deployable in at least a radial outward direction relative to the longitudinal axis of the housing to engage respective opposed arterial tissue portions on opposed sides of the opening and move the tissue arterial portions to an everted condition thereof. The first and second jaw members are adapted for relative movement between an open position to facilitate positioning about the arterial tissue portions in the everted condition and a closed position to at least partially draw the arterial tissue portions together to an at least partial approximated condition. An electrode is associated with at least one of the first and second jaw members and arranged to contact the respective arterial tissue portions. The electrode is adapted to be connected to a radio frequency energy source whereby energy is transmitted through the electrode to thermally fuse the arterial tissue positions between the first and second jaw members to substantially close the opening. Preferably, an electrode is associated with each of the first and second jaw members. Each electrode nay be configured as a bipolar electrode.

    摘要: An apparatus for facilitating closure of an opening in a blood vessel, includes a closure instrument having an elongated member defining a longitudinal axis and proximal and distal ends. The elongated member has a vacuum lumen extending at least a portion of the length thereof for conveying a vacuum and terminating in a vacuum port adjacent the distal end of the elongated member. The distal end of the elongated member is dimensioned to be positioned proximal a vessel opening in a blood vessel whereby vessel edge portions defining the vessel opening are at least drawn toward the vacuum port in response to a vacuum conveyed through the vacuum lumen. At least one surgical clip is mounted adjacent the distal end of the elongated member and is adapted to be formed to an at least partially formed condition thereof. The one clip is positioned with respect to the vacuum port to engage the vessel edge portions drawn toward the vacuum port upon movement of the one clip to the formed condition thereof to generally approximate the vessel edge portions to at least partially close the vessel opening. A method for facilitating closure of an opening in a blood vessel is also disclosed.

    摘要: A surgical tool and method of operation are provided. The surgical tool includes an end effector, such as a surgical scissor. The end effector is coupled to an actuating mechanism having a clevis and a pulley. A cable engages the pulley and is attached to one side of the clevis. When one portion of the cable is pulled, the clevis is moved in a first direction causing the end effector to open. When a second portion of the cable is pulled, the clevis moves in the opposite direction and the end effector is closed. The system also includes a mechanism for changing the orientation of the end effector. Another embodiment includes a mechanism for actuating the cable and the end effector.

In use, after loading of the loading unit in the surgical fastener applying apparatus and subsequent clamping of the cartridge and anvil half-sections, the lockout mechanism is moved to an unlocked position to allow advancement of the member.

    摘要: An improved surgical fastener applying apparatus includes a cartridge half-section and an anvil half-section with the cartridge and anvil half-sections being relatively movable from an unclamped position to a fully clamped position. The apparatus includes a replaceable staple cartridge assembly having a plurality of surgical staples and a pivotably mounted safety lock-out. The safety lock-out is pivotable between an initial position in which a relative movement of the cartridge and anvil half-sections to the fully clamped position is permitted and a blocking position in which a relative movement of the cartridge and anvil half-sections to return them to their fully clamped position after the plurality of staples have been at least partially fired is prevented. The apparatus further includes a pivotably mounted firing lever for moving a pair of camming surfaces longitudinally. In addition, each of the presently disclosed cartridge and anvil half-sections of the fastener applying apparatus includes a clamping lever, which clamping levers are configured and adapted to enable the applying apparatus to be opened by either one of the cartridge half-section and the anvil half-section.
