An apparatus for forming a concrete slab to be supported on a surface of load bearing members, and a method of use of the apparatus, are provided. The apparatus may includes a support element having a layer of active material bonded on each side by a backing sheet. The active material is chosen to slowly compress over time when placed under a load of the concrete when poured onto the apparatus. The rate of compression is chosen such that the layer of active material continues to support the slab until such time as the slab becomes self-supporting (i.e., the concrete has cured/set) on the load bearing members. The active material is chosen such that the load supported by the active material reduces gradually from an original value of 100% when the concrete is poured to less than 40% of its original value after 10 days as the concrete cures. Another feature is the inclusion of a surface of the backing sheet with a relatively low coefficient of friction. When two support elements are laid one on top of the other with the surfaces in contact, the upper support element may slide over the lower support element, for example during a seismic event.

In a particular embodiment the form includes a cardboard box beam having a depressed longitudinal section along an outer portion of its length for added structural integrity. A lateral notch is disposed in the outer portion of the form at intervals and across a width of the form to divide the form into what will be the sides of the form when assembled. The lateral notches include a flexible planar connector of an inner portion of the form to allow the notch to act as a hinge and bend to create each corner of the form. A tab extends from one end of the form and is used to tuck in an opposing end of the form. The surfaces of the form may be coated with wax, or other sealant, to prevent moisture damage to the form as the concrete is curing.

The cellular structure is impervious to moisture. Together the cellular structure and the shell make a singular rigid member. The rigid member is configured to temporarily support concrete and gradually increase in flexure so as to flex with the movement of the expansive soil. The shell is configured to weaken over time from the regulated absorption of moisture. The flexure of the cellular structure configured to increase with the deterioration of the shell.
