摘要： A test booklet and method of administering a test used to receive instant continuing education credit. The test booklet includes a number of multiple choice questions where each question includes a plurality of answer selections. The question designations and answer selections are reproduced on an answer sheet that is attached to the booklet by a perforation. Each answer selection is covered by an opaque scratch-off material such that the applicant selects his or her answer by scratching off the appropriate material covering a particular answer selection. To receive the continuing education credit, the applicant tears the answer sheet from the booklet and mails in the answer sheet to an accreditation agency to be graded.

摘要： Instructional apparatus employs a holding mechanism which has depressions in its upper surface of either one depth or different depths. A printed format containing pre-punched openings representing answer selections is disposed over the upper surface, and some or all of the openings are aligned with corresponding depressions. Sandwiched between the printed format and the upper surface is a highly elastic sheet of rubber-like material. The user pushes a rounded tip stylus through the selected opening and thence into the elastic sheet, and the degree of penetration by the stylus through the upper surface down into an underlying depression or hole indicates the correctness of the response and provides instantaneous feedback. Upon withdrawal of the stylus the elastic sheet returns to its former shape with no deformation; thus the apparatus may be used by many students without replacing formats or other used components.

摘要： This invention consists of a template for use with a readable answer sheet. The answer sheet records the answers to multiple-choice, true-false or matching questions and includes answer blocks containing four columns, although at times, only two columns may be utilized by the testee as in the case of answering true-false questions. The template includes a base portion provided with openings and means to receive an answer plate or card on each of the openings so that the answer sheet may be checked and marked without resort to a machine to read the answers. The openings are provided with means to maintain the answer plate or card in registry with the answer blocks on the answer sheet so as to permit the visual inspection of the answers through apertures provided in the answer card.

摘要： A self-correction answer and scoring sheet for use in multiple choice testing. The sheet is imprinted on the same surface both with a panel of groups of response boxes to be marked by the person tested and a scoring panel having scoring boxes which correspond to the correct or preferred answer for each group of response boxes. The sheet is folded so that the scoring panel underlies the response panel with each of the scoring boxes being in registry with a selected response box in each group, the selected response box corresponding to the correct or desired answer. Means are provided for forming a visible mark on the score sheet in response to impression of an answer on the response panel. A correct response is indicated by the formation of the visible mark within the score box. An incorrect answer is indicated by the formation of the visible mark outside of the score box.
