A textile-based surgical implant that can be inserted through any number of suitable surgical approaches, including but not limited to lateral, anterior, anterior-lateral, posterolateral, and/or posterior approaches. When applied to spine surgery and inserted into an intervertebral disc space, the implant restores the normal height of the intervertebral disc space, while advantageously preserving the natural motion of the spine. The textile construction of the implant is beneficial because it is generally compliant and thereby restores and/or improves spinal motion. The compliant nature of the textile-based implant provides the required flexibility and elasticity to advantageously support physiological movements, as opposed to fusion surgery which forms a boney bridge between adjacent vertebral bodies. In addition, the porosity and biocompatibility of the textile-based implant facilitates tissue ingrowth throughout part or all of the implant, which helps to secure and encapsulate the implant in the intervertebral space.

A core may be packed in the enclosure. The core may be compressible and/or flexible. In certain embodiments, the core may include a plurality of elongated members (24). In other embodiments, the core may include ridges or grooves. The enclosure may be placed in an intervertebral space between the adjacent vertebrae such that the core is disposed between the adjacent vertebrae. Constructs that are at least partially constructed of allograft cancellous bone are disclosed, along with cartilage particles that may be used with the constructs for repairing articular cartilage defects. A multi-piece construct (20) includes a base member (22), a cap member (30) and at least one pin (40) that secures the cap member (30) to the base member (20). The base member (22) may be constructed of mineralized cancellous bone, and is used to replace the subchondral bone removed when a surgeon cuts a bore in the area of an adjacent cartilage defect. The base member (22) includes a blind bore (23) and first and second through-going transverse bores (28, 29) in opposite sides of a wall of the base member (22). The cap member (30) includes an upper section (32) that has a thickness that is similar to that of a patient's surrounding articular cartilage layer and a stem (36) depending from the upper section (32) that is dimensioned to be received in and by the blind bore (23) of the base member (22). The stem (36) includes a transverse through-going bore (37), which may be aligned with the transverse through-going bores (28, 29) of the base member (22) to receive the pin (40) therein when the construct (20) has been assembled. The cap member (30) is at least partially formed of demineralized allograft cancellous bone, into which a mixture containing lyophilized, freeze-milled allograft cartilage particles may be infused for the repair of articular cartilage defects. The cartilage particles have a size within a range of from about 10 microns to about 210 microns.

Materials and methods for repairing or replacing an intervertebral disc or disc nucleus using natural biological tissue (11). The tissue may be used alone without an additional core material, or it may be used to encapsulate an elastomeric or hydrogel core. When used alone the tissue may be rolled, folded, braided, or layered to provide a solid plug of natural biological material. When used to encapsulate an elastomeric or hydrogel core the tissue acts as a constraining jacket to support the core. The natural tissue implants (10) may further include a drawstring to assist in folding the implant when the device is implanted in an intervertebral disc space. Multiple units of the natural tissue implants may be used together.
