摘要: A surgical access device is adapted for performing laparoscopic surgical procedures with at least one instrument passing through the surgical access device and through an incision in the abdominal wall of a patient with the abdominal cavity pressurized with an insufflation gas. The surgical access device is adapted to provide instrument access to the abdominal cavity for surgical procedures while generally maintaining insufflation pressure in the abdominal cavity. The surgical access device comprises an access seal. The access seal comprises a material formed of a mixture comprising a triblock copolymer and an oil. The access seal is adapted to be disposed relative to the abdominal wall in an operative position. The material of the access seal is adapted to form a seal to generally maintain insufflation pressure within the abdominal cavity. At least one opening is formed through the material of the access seal between a proximal portion and a distal portion of the access seal. The at least one opening when operatively disposed is in communication with the incision and forms a working channel between a location external to the abdominal wall and a location internal to the abdominal wall. The material of the access seal is adapted to conform to a surface of an instrument inserted through the working channel.

摘要: A robotic surgical system is configured with rigid, curved cannulas that extend through the same opening into a patient's body. Surgical instruments with passively flexible shafts extend through the curved cannulas. The cannulas are oriented to direct the instruments towards a surgical site. Various port features that support the curved cannulas within the single opening are disclosed. Cannula support fixtures that support the cannulas during insertion into the single opening and mounting to robotic manipulators are disclosed. The support fixtures include arms that swing in an arc to insert a curved cannula, repositionable arms coupled to a stable cannula base, and an insertion cap that holds a cannula. A teleoperation control system that moves the curved cannulas and their associated instruments in a manner that allows a surgeon to experience intuitive control is disclosed.

摘要: An incrementally adjustable wound retractor, which provides access to a body cavity, includes an inner ring having a diameter greater than the desired diameter of the wound incision, an outer ring having an annular axis and a diameter greater than the desired diameter of the wound incision, and a flexible sleeve disposed in a generally cylindrical form between the inner and outer rings. The outer ring includes first and second circular tubes spaced apart axially with each including a lumen having a rigid, noncompliant split hoop placed therein. The outer ring may be rolled over itself and around the annular axis to retract the sleeve with sufficient force to stretch the incision to the desired diameter. A gel cap seal may be coupled to the outer ring outside of the biological body to seal the opening produced by the wound retractor between the body cavity and outside the body cavity.

摘要: A surgical access assembly includes a cannula, a housing mounted to the cannula, and a seal disposed within the cannula. The cannula is adapted for insertion through tissue to permit access to an underlying tissue site. The cannula defines a longitudinal axis and has a longitudinal passageway for passage of a surgical object for positioning with respect to the tissue site. The housing is adapted for movement relative to the cannula in a direction transverse to the longitudinal axis. The seal has a passage for reception of the surgical object in substantial sealed relation therewith. The access assembly may further include a support member mounted to the cannula. The support member adapted to maintain a predetermined distance between the housing and the tissue. The support member may be securely affixed to, or selectively positionable along, the cannula with respect to the longitudinal axis.

摘要: A trocar for use in a minimally invasive surgical procedure includes an elongated body, nozzle means and means for delivering a pressurized flow of fluid to the nozzle means. The elongated body has a generally tubular configuration with coaxially arranged inner and outer walls and longitudinally opposed proximal and distal end portions, with the inner wall defining a lumen to accommodate passage of an instrument therethrough. The nozzle means is operatively associated with the inner wall of the body for directing pressurized fluid into the lumen to develop a pressure differential in an area within a region extending from a location adjacent a distal end portion of the lumen to a location adjacent a proximal end portion of the lumen, to form a fluid seal around an instrument passing therethrough.

摘要: Methods and devices are provided to improve access to a surgical site during surgical procedures. In one exemplary embodiment a surgical access device is provided having a cannula, a cannula base, and a retractor. The cannula base is configured to couple both to the retractor and the cannula to provide access to a surgical site. The cannula can be removably and replaceably coupled to the cannula base. Optionally, the cannula base can be removably and replaceably coupled to the retractor. As desired, the cannula can be decoupled from the cannula base, which can allow for an object to be removed from the surgical site and/or for the cannula, or another cannula to be attached to the cannula base that remains in place to maintain access to the surgical site. Exemplary methods for accessing a surgical site are also provided.

摘要: A surgical access apparatus for use during a surgical procedure for passing through tissue to an underlying surgical area is provided. The surgical access apparatus includes a vibration source connected to a seal member located in an internal passage of an access member. The seal is configured to form a substantial sealing relation with an inserted surgical instrument and substantially close in the absence of the surgical instrument. The vibration source causes the seal to vibrate, in order to decrease the force needed to insert the surgical instrument. The source of vibration may be one of the following: a rotating eccentric mass, mass moving in a linear path, exciting a piezoelectric, a crystal, or an electro active polymer capable of expanding or contracting.

摘要: A surgical access portal includes a seal housing, a sleeve mountable to the seal housing, and a seal disposed within the seal housing. The seal includes a first layer having an opening for the reception and passage of a surgical instrument, a second continuous layer for providing a fluid tight seal in the seal housing in the presence and/or in the absence of a surgical instrument, and a third layer having an opening for the passage of a surgical instrument therethrough. The first and third layers have a higher density than the second layer for maintaining the second layer in an axially compressed state therebetween.

摘要: A seal device having a substantially conical shape including a slit opening at its tip defined by a pair of planar surfaces, the opening reinforced by gussets on the exterior surface of the seal device and adjacent to the end regions of the slit. The gussets help bias the ends of the slit towards a closed position, even when an instrument is located within the slit, to thereby reduce “cat-eye”-ing and minimize leakage therethrough. The seal device may include ramps on respective inner surfaces of the device, the ramps functioning as a lead-in to the slitted opening such an inserted instrument contacts the ramps first and opens the slit prior to the instrument contacting the material of the slit. Along the outer surface of the device is a flexible body portion to minimize the effect on the opening when an instrument is inserted into the device.
