    摘要: A system and method for determining patient-specific implantable medical device programming parameters are presented. A set of physiological measures collected through a plurality of sensors monitoring physiological functions in a patient are assimilated. The physiological measures set are analyzed to identify physiological idiosyncrasies specific to the patient and to optimize the therapy to be delivered through an implantable medical device. A patient profile is formed including a plurality of programming parameters and the patient profile is preprogrammed into the implantable medical device prior to implantation in the patient.

The device is inserted into the venous system, the balloon is inflated and the device is floated along a blood flow path within the venous system through the heart and into the pulmonary artery. The lead is delivered into the pulmonary artery using the device. In one embodiment, the device is a catheter that facilitates placement of an implantation catheter and/or a guide wire into the pulmonary artery for delivery of the lead. In another embodiment, the lead is mounted over the catheter. In yet another embodiment, the delivery device is formed at the distal end of the lead.

An implantable baroreflex activation system includes a control system having an implantable housing, an electrical lead, attachable to the control system, and an electrode structure. The electrode structure is near one end of the electrical lead, and includes a monopolar electrode, a backing material having an effective surface area larger than the electrode, and a releasable pivotable interface to mate with an implant tool. The electrode is configured for implantation on an outer surface of a blood vessel and the control system is programmed to deliver a baroreflex therapy via the monopolar electrode to a baroreceptor within a wall of the blood vessel.

In one example, the activation/deactivation and/or change to the parameters of the rate smoothing function are temporary, and the pre-event state of the rate smoothing function is set to a post-adjusting state, such as after a first time interval. Rate smoothing may be selected, activated or deactivated, or adjusted based on a cardiac signal state, or based on an activity or other physiological sensor signal. The adjusted rate smoothing parameters may include an up-smoothing percentage to limit a speed of pacing rate increase and a down-smoothing percentage to limit a speed of pacing rate drop.
