    摘要: A valvulotome for disrupting valve cusps in the vein of a patient includes a shaft and a cutting member which extends along an axis that defines an axial direction as well as an angular direction. First and second portions of the cutting member define respective cutting edges which extend outwardly of the axis in an angular relationship and an axial relationship with each other. The first and second cutting edges engage and disrupt different cusps of the valve when the shaft is withdrawn axially from the vein. Each of the cutting edges is protected by a proximate shoulder which inhibits the engagement of sidebranches by the associated cutting edge. The cutting member may be formed as a helical blade, a wire or a cylinder and may be provided with a swivel coupling as well as a sheath operable to cover or expose the cutting member.

    摘要: A system for facilitating hemostasis of a puncture site in a blood vessel injects an absorbable sponge pledget in a hydrated state to a position at an exterior of the blood vessel puncture to facilitate hemostasis. The system includes a tract dilator for locating the puncture, an introducer, and a pusher. The introducer includes a staging chamber for receiving the absorbent sponge pledget and a delivery chamber having a diameter smaller than a diameter of the staging chamber into which a hydrated pledget passes. The pledget is hydrated and compressed within the introducer and then ejected with the pusher to the delivery site to facilitate hemostasis of the blood vessel puncture. The system delivers the absorbable sponge pledget over a guidewire to ensure the proper positioning of the pledget over the puncture. The implanted absorbable sponge once delivered expands rapidly to fill the target site and is absorbed by the body over time.

    摘要: A method for performing a surgical operation on a body conduit includes the step of inserting into the conduit an apparatus having at its distal end an element the location of which is detectable from regions exterior of the body. The apparatus is moved within the conduit until the element is positioned relative to a predetermined location where the element functions as a target. Detecting the location of this target within the conduit facilitates insertion of a surgical instrument percutaneously of the body and toward the predetermined location. The targeting apparatus may be used with an endoscope and may include a variety of active and passive elements which form the target. The surgical instrument can be inserted percutaneously transverse to the axis of the conduit to perform a variety of surgical operations. Sidebranch occlusion in an insitu procedure is greatly facilitated by this method and apparatus.

    摘要: An applicator for delivering brachytherapy includes elongate members movable between collapsed and expanded configurations for delivering brachytherapy within a lumpectomy cavity, a vaginal cavity, or other target region. The elongate members may be expandable into a symmetrical or asymmetrical expanded configuration, e.g., into a generally spherical, pear-shaped, or planar configuration. A system for delivering brachytherapy includes the applicator and an access device for lining and/or dilating a body cavity and/or for receiving the applicator therein. The access device is advanced into a body cavity, an expandable member on the access device is inflated, the applicator is advanced into the access device, and the elongate members are expanded to deliver radiation to the target region. Alternatively, the access device carries an expandable device into the target region, the expandable device is removed after dilating the target region, and the applicator is introduced through the access device to deliver radiation.

One or more helical catheters are provided on a distal portion of the elongate body, e.g., including distal ends coupled to the core member and proximal ends coupled to the outer member. The outer member is movable relative to the core member, e.g., axially and/or rotationally, to direct the helical catheters between a collapsed configuration for introduction through a tissue tract to a target location, e.g., a lumpectomy cavity, and an expanded configuration. Each helical catheter includes a lumen, and, after expansion to the expanded configuration, a source of radiation may be introduced along the helical catheters and/or a lumen of the core member for delivering radiation to the target location. Optionally, the apparatus may include a balloon surrounding or within the helical members.
