Laser treatment of tissue, particularly the tissues in or around the nasal and oral cavities, are described herein. One method for reducing the size of the tissue being treated is to apply laser energy to the underlying tissue. One instrument may be used to deliver laser energy and to optionally provide an infusion or injection of a fluid directly into the tissue as well as optionally provide for ultrasound energy application as well. One or more optical fibers which may extend through needles inserted into the tissue may be utilized to deliver the laser energy.

The distal end of the optical fiber and the hollow tube are configured so as to emit, by refraction (total internal reflection) or reflection from a metal surface, laser energy at an angle of about 80° to about 90° relative to the longitudinal axis of the optical fiber and hollow tube. Improved laser energy delivery devices, providing greater transmission efficiency, longevity of use and safety are described. In one embodiment, a device for delivering laser energy includes a conventional optical fiber within a metal or plastic cannula provided with a laser energy emission window. A reflective material is disposed in a space between the distal end portion of a metal or plastic sheath and the optical fiber, as well as about any extension or endpiece of the distal end of the sheath. The reflective material reflects aberrant emissions of laser energy from the optical fiber as it erodes, and laser energy backscattered from the target tissue away from the distal end of the sheath back into the optical fiber or out of the distal end of the sheath, thereby preventing damage due to overheating. Other embodiments include devices that transmit laser energy more safely, efficiently and durably laterally from the axis of the optical fiber.

The present invention contemplates a surgical device for both ablating a channel in a patient's tissue and also delivering a therapeutic agent. The device includes an elongated multi-lumen tube, an elongated tissue ablating assembly, and a therapeutic agent delivery assembly. The therapeutic agent is capable of being delivered into the channel and/or to the surrounding tissue. The device may further include a second multi-lumen tube also capable of delivering therapeutic agents. Although suitable for many operations, the device is particularly well suited for transmyocardial revascularization operations. The present invention also contemplates a procedure for using such a surgical device to ablate a channel in a patient's tissue and also deliver a therapeutic agent.

The elongate device carries the beam through an optical fiber that has a chamfered end for deflecting the beam out of a distal end of the device at an angle. Tissue coagulation is monitored using a magnetic resonance imaging system that provides periodic tissue temperature data. A small fluid supply duct provides cooling fluid that expands to a gas near the distal end of the device, thereby cooling the tip.

Apparatus and method for performing interstitial laser therapy and adjunctive therapy on a patient are revealed. The apparatus employs a combination of a mammography unit, an interstitial laser treatment device attached to the mammography unit and a treatment platform positioned relative to the mammography unit to enable the interstitial laser therapy to be performed. The use of the treatment platform with the mammography unit enables the interstitial laser therapy to be performed and, if necessary, adjunctive therapy to be performed in the same treatment room without transferring the patient to a new platform.
