摘要： A surgical instrument for use with a surgical portal apparatus includes an elongate shaft having proximal and distal ends and defining a longitudinal axis and a dilating member disposed at the distal end of the elongate shaft. The dilating member has a first longitudinal arc segment and a second longitudinal arc segment in diametrical opposed relation to the first longitudinal arc segment. The first and second longitudinal arc segments define respective first and second radii of curvature transverse to the longitudinal axis. The first and second radii of curvature decrease from proximal to distal to define a generally tapered configuration of the dilating member. The first radius of curvature is greater than the second radius of curvature at predefined longitudinal positions of the dilating member such that the first longitudinal arc segment extends distally beyond the second longitudinal segment to define a penetrating tip. The penetrating tip has a reduced profile and is dimensioned to facilitate initial insertion of the dilating member into tissue.

Each ferrule holder is configured to releasably retain a ferrule therein. Each ferrule is configured to retain a portion of the suture thereon. The ferrule assembly is rotatable between a first position and a second position. The guide lumen(s) are configured to direct a suture passer into engagement with the ferrule of one of the ferrule holders when the ferrule assembly is in one of the first and second positions.

The annular element is dimensioned to minimize offset manipulation of the surgical object relative to the longitudinal axis.

摘要： Accordingly, a surgical portal apparatus for accessing a cavity is provided. The surgical portal apparatus includes a portal member defining a longitudinal axis and having a longitudinal passageway therethrough for receiving a surgical object, and an adjustable seal disposed within the longitudinal passageway. The adjustable seal includes a first configuration wherein the adjustable seal defines a first opening configured for receiving a first surgical object having a first diameter. The first opening defines a first central axis therethrough. The adjustable seal includes a second configuration wherein the adjustable seal defines a second opening configured for receiving a second surgical object having a second diameter. The second opening defines a second central axis laterally offset from the first central axis. The apparatus further including an adjustment member mounted within the portal member and positioned to intersect the longitudinal passageway to engage the first or second surgical object and move relative to the portal member. The adjustment member is operatively connected to the adjustment seal to move the adjustable seal to the first configuration upon engagement with a first surgical object to thereby facilitate passage of the first surgical object through the adjustable seal.

The inner guide portion defines a channel adapted to generally direct the surgical instrument toward the inner seal portion of the seal upon entry of the surgical instrument within the longitudinal opening of the seal housing. The inner guide portion is relatively compliant to accommodate initial offset or angled entry of the surgical instrument.

The manual lock member is preferably adapted for rotational movement relative to a longitudinal axis of the seal housing to move between the first and second positions thereof.
