    摘要: An electrosurgical system and method for treating hard and soft tissues in the body comprises a shaft, a distal end section, an active electrode associated with the distal end section, a first fluid supply adapted to deliver a first electrically conductive fluid to the target site, and a second fluid supply adapted to deliver a second electrically conductive fluid to the active electrode. An instrument of the invention includes a rotating member housed longitudinally within a shaft, a tissue removal port disposed at the shaft distal end portion, and an active electrode disposed at the instrument distal end. The active electrode is adapted to electrosurgically remove at least a portion of the target tissue as the rotating member rotates within the shaft. According to alternative embodiments, the active electrode may be disposed on the rotating member or on the shaft.

The electrosurgical device and related methods are provided for resecting, cutting, partially ablating, aspirating or otherwise removing tissue from a target site, and ablating the tissue in situ. The present methods and systems are particularly useful for removing tissue within joints, e.g., synovial tissue, meniscus, articular cartilage and the like.

    摘要: An electrosurgical system and method for ablating, resecting, or cutting body structures, with minimal or no damage to tissue adjacent to the treatment site. The system includes an electrosurgical probe having a shaft with a shaft distal end bifurcated to provide first and second arms. First and second electrode supports are disposed on the first and second arms, respectively. At least one active electrode, in the form of a loop or partial loop, is arranged between the first and second electrode supports. A return electrode, also in the form of a loop or partial loop, is arranged between the first and second electrode supports distal to the active electrode. The active and return electrodes are configured to promote substantially high electric field intensities and associated high current densities between the active portion and the target site when a high frequency voltage is applied to the electrodes. These high current densities are sufficient to break down the tissue by processes including molecular dissociation of tissue components. In one embodiment, the high frequency voltage imparts energy to the target site to effect the vaporization and volumetric removal of a layer of tissue without causing substantial tissue damage beyond the layer of tissue ablated. In another embodiment, a fragment of target tissue is removed, with minimal or no damage to surrounding tissue, by a process including the molecular dissociation of tissue components, and the tissue fragment is retrieved for biopsy.

    摘要: A medical electrical lead for establishing an electrical connection with a tissue of the body, the lead having a lead body and a semi-cylindrical cuff. In the preferred embodiment the semi-cylindrical cuff features one or more electrodes. The semi-cylindrical cuff having a long flap which wraps about the cuff and a short flap which wraps about the long flap. The semi-cylindrical cuff is relatively stiff as compared to the short flap. The short flap, in turn, is relatively the same stiffness as the long flap. The stiffness of each flap may be varied, however, so that one is more or less than the other, and in turn, than the cuff. Through such a multi-flap construction the electrodes may be positioned proximal to a nerve in such a manner that the mechanically induced damage may be minimized or even entirely eliminated. Finally, a method of implanting such an electrode is also disclosed.

    摘要: Apparatus and methods for ablating, severing, cutting, shrinking, coagulating, or otherwise modifying a target tissue to be treated. In a method for treating a target tissue, an active electrode of an electrosurgical probe is positioned in at least close proximity to the target tissue in the presence of an electrically conductive fluid. A high frequency voltage is then applied between the active electrode and a return electrode, wherein, the high frequency voltage is sufficient to volumetrically remove (ablate), sever, or modify at least a portion of the target tissue. The probe comprises a multi-lumen shaft having a plurality of internal lumens, and a return electrode coil oriented substantially parallel to the shaft distal end. The active electrode may be in the form of a metal disc, a hook, or an active electrode coil. In the latter embodiment, the active electrode coil is typically arranged substantially orthogonal to the return electrode coil. Methods of making an active electrode coil, a return electrode coil, and an electrosurgical probe are also disclosed.
