摘要: A pelvic organ prolapse treatment device includes an implantable support sized to support and treat a prolapsed organ of a patient, a suture, a knot separate from the suture, and a dilator. The implantable support includes an arm having a first portion connected to a body of the implantable support and an end portion extending away from the body of the implantable support. The end portion of the arm is folded to include a first fold and a second fold that combine to reduce a lateral dimension of a portion of the end portion of the arm. The suture extends away from the end portion of the arm. The knot is provided separately from the suture and is secured around the first fold and the second fold to capture both the end portion of the arm and the suture. The dilator is disposed over and connected to the knot.

摘要: Apparatus and methods for positioning and securing anchors are disclosed herein. The anchors are adapted to be delivered and implanted into or upon tissue, particularly tissue within the gastrointestinal system of a patient. The anchor is adapted to slide uni-directionally over suture such that a tissue plication may be cinched between anchors. A locking mechanism, either within the anchor itself or positioned proximally of the anchor, may allow for the uni-directional translation of the anchor while enabling the anchor to be locked onto the suture if the anchor is pulled, pushed, or otherwise urged in the opposite direction along the suture. This uni-directional anchor locking mechanism facilitates cinching of the tissue plication between the anchors, and it may be utilized in one or several anchors in cinching a tissue fold.

摘要: Methods and apparatus for providing transapical access to a heart chamber for performing an intra cardiac procedure are described. The apparatus include a helical needle driver, a dilator, a straight access needle, and optionally a guidewire. After entering the heart chamber with the straight access needle, the helical needle driver is used to place a helical suture within the myocardium. After removing the needle driver, the dilator is advanced through the pre-placed helical suture, dilating both a passage and the circumscribing suture. After performing procedure, the pre-placed suture may be closed by proximally retracting an external end of the suture.

摘要: One aspect of the present disclosure relates to a cannula for passing a tissue graft through soft tissue. The cannula can comprise an elongated body including oppositely disposed first and second ends and an axis that extends between the first and second ends. The elongated body can further include a passage extending between the first and second ends. The second end can include a curved portion that terminates at a sharpened, pointed tip configured to pierce and/or cut soft tissue at the surgical repair site. The tip can be axially offset from the axis of the elongated body at an angle that is less than 180°.

摘要: An apparatus for remotely and subcutaneously positioning a tethering strand in tissue includes a housing, an actuator coupled with the housing for longitudinal reciprocation relative to the housing, and a nested pair of needles including a first needle coupled with the housing and a second needle coupled with the actuator. One of the needles has a sharpened tip and another one of the needles has a blunted tip. The first needle has a first length and the second needle has a second length, and the first length and the second length are selected so that advancement of the second needle using the actuator causes the sharpened tip to project beyond the blunted tip, and so that retraction of the second needle using the actuator causes the blunted tip to project beyond the sharpened tip.

摘要: A suturing device includes a handle, an elongated body, at least one suture snag, at least one pair of needles, and at least one suture pair. The suture snag is moveable between a deployed position in which two distal arm portions thereof radially extend away from the elongated body and a retracted position in which the two distal arm portions are disposed within the elongated body. The suture pair is slidingly disposed through the needle pair. The suturing device deploys the suture snag within a vessel adjacent to an arteriotomy, extends the needle pair through a vessel wall around the arteriotomy and through the deployed suture snag, extends the suture pair beyond the distal ends of the needle pair, and then utilizes the suture snag to capture the extended suture pair by retracting the suture snag to pull first or distal ends of the sutures back into the suturing device.

摘要: Embodiments of invention involve tissue approximation instruments that may be delivered to the body of a patient during minimally invasive or other surgical procedures. In one group of embodiments, the instrument has an elongated configuration with two sets of expandable wings that each have spreadable wings that can be made to expand when located on opposite sides of a distal tissue region and a proximal tissue region and can then be made to move toward one another to bring the two tissue regions into a more proximate position. The instrument is delivered through a needle or catheter and is controlled by relative movement of a push tube and control wire wherein the control wire can be released from the instrument via rotation in a first direction and can cause release of the approximation device from tissue that it is holding by rotation in the opposite direction.

摘要: Method and apparatus for limiting absorption of food products in specific parts of the digestive system is presented. A gastrointestinal implant device is anchored in the stomach and extends beyond the ligament of Treitz. All food exiting the stomach is funneled through the device. The gastrointestinal device includes an anchor for anchoring the device to the stomach and a flexible sleeve to limit absorption of nutrients in the duodenum. The anchor is collapsible for endoscopic delivery and removal.
