The subject invention relates to the detection, diagnosis and risk stratification of clinical events such as acute coronary syndrome, in patients with signs and symptoms of suspected cardiac origin. In one embodiment, a clinical event in a patient is diagnosed by obtaining the patient's ECG, and at least one in vitro diagnostic assay, preferably an assay for a marker of ischemia, and optionally in vitro diagnostic assays for necrotic markers or other cardiac indicators, and combining the foregoing results in an algorithm to provide a diagnosis or a risk stratification of the clinical condition.

Line scans are taken through the upper and lower regions of the text line. Whichever group has the higher average density is the group associated with the tops of the characters. To print the document, data is read out of the memory in the order in which it was received, or in the reverse order, such that an "up side down" document is reoriented. A determination of the order in which printing should take place is made without requiring the actual recognition of any characters.

A cochlear implant system includes an electrode array (1) comprising multiple platinum ring electrodes in a silastic carrier to be implanted in the cochlea of the ear. A receiver-stimulator (3) containing a semiconductor integrated circuit and other components is implanted in the patient adjacent the ear to receive data information and power through tuned coil (5) using an inductive link (6) from a patient-wearable external speech processor (7) including an integrated circuit and various components which is configured or mapped to emit data signals from an EPROM programmed to suit each patient electrical stimulation perceptions through testing of the patient and his implanted stimulator/electrode using a diagnostic and programming unit (12) connected to the processor by an interface unit (10). The system allows use of various speech processing strategies including dominant spectral peak and amplitude and compression of voice pitch so as to include voiced sounds, unvoiced glottal sounds and prosodic information. Biphastic pulses are supplied to various combinations of electrodes by a switch controlled current sink in various modes of operation. In-place testing of the implant is also provided. Various safety features are incorporated to insure that harmful impulses are not imposed on the patient.Transmission of data is by a series of discrete data bursts which represent the chosen electrode(s), the electrode mode configuration, the stimulating current, and amplitude determined by the duration of the amplitude burst.

In a cardiomyoplasty system, a rate responsive implantable device is disclosed which controls the stimulation of both the heart and a myoplasty muscle. The metabolic demand is used as a rate response parameter and one or both of the heart and the muscle stimulation are increased with metabolic demand. Under certain conditions, for example, if the muscle is tired, its stimulation is maintained constant or reduced and the cardiac stimulation is used to compensate for increased metabolic demand.

The system utilizes a series of pacing pulses of progressively decreasing energies to stimulate the ventricle, and detects evoked responses through measurements of the integrals of the R-waves provided by a surface electrocardiogram. Upon loss of capture, the minimum pacing energy is a function of the energy of the last pacing pulse which evoked a ventricular depolarization.
