A tissue retaining ring for use with a plurality of surgical fasteners mounted within a disposable loading unit utilized during a vascular anastomosis inlcudes a genrally circular, semi-pliable material having a series of alternating loops and arcuate portions. Each of the loops defines an aperture therein which is dimensioned to receive a distal end of one of the plurality of surgical fasteners. When the instrument is fired, the surgical fasteners are simultaneously deformed through the tissue ring and through the tissue. The tissue retaining ring holds the tissue in place along the surgical fastener and reduces the chances of tissue slippage after deformation.

The graft attachment assembly includes an attachment member including a base portion having a convex top surface and a branch portion having a passageway therethrough. The branch portion projects outwardly from the base portion. A clamp member having a concave bottom surface is configured to sealingly engage the top surface of the base portion and an opening is dimensioned to slidably receive the branch portion. The clamp member is slidable about the branch portion to a position adjacent the base portion to clamp tissue therebetween. A locking member is slidable about the branch portion and dimensioned to secure a vessel thereabout.

There is disclosed a surgical instrument and loading unit for performing an anastomosis of first and second blood vessels. The surgical instrument includes a handle assembly and a body portion extending distally from the handle assembly. The detachable loading unit includes an anvil releasably engagable with a distal end of the body portion and having a plurality of fasteners disposed in channels about the circumference of a distal end of the anvil. A pusher is slidably mounted over the anvil such that distal movement of the pusher in response to actuation of the handle assembly cams the fasteners between the anvil and a distal end of the pusher to simultaneously crimp the fasteners about tissue. A method of performing an anastomosis is also disclosed and includes positioning a first vessel in the loading unit and partially inserting the first vessel through an opening in a second vessel. The surgical instrument is subsequently attached to the loading unit and the handle assembly is actuated to crimp fasteners about the first and second vessels to form an anastomosis.

Also provided is a method of performing a transvaginal surgery. The method includes providing an access port including a retention mechanism configured to selectively retain the access port within the vagina of a patient, forming an incision in the wall of the vagina into the abdomen of the patient, inserting a distal end of the access port into the vagina and through the incision, and activating the retention mechanism to secure the distal end of the access port within the abdominal cavity of the patient.
