    摘要: A technique is provided for detecting episodes of cardiac ischemia based on an examination of the total energy of T-waves. Since cardiac ischemia is often a precursor to acute myocardial infarction (AMI) or ventricular fibrillation (VF), the technique thereby provides a method for predicting the possible onset of AMI or VF. Briefly, the technique integrates internal electrical cardiac signals occurring during T-waves and then compares the result against a running average. If the result exceeds the average by some predetermined amount, ischemia is thereby detected and a warning signal is provided to the patient. The maximum slope of the T-wave is also exploited. Techniques are also set forth herein for reliably detecting T-waves, which help prevent P-waves from being misinterpreted as T-waves on unipolar sensing channels. The T-wave detection technique may be used in conjunction with ischemia detection or for other purposes.

    摘要: An implantable cardiac stimulation lead system for use with a stimulation device includes a lead body preferably having an outer diameter of less than 4 French and formed of a biocompatible material having a Young's modulus of at least 20,000 pounds per square inch. In the lead body, an inner insulator is composed of a suitable polymer such as PTFE or ETFE with an outer peripheral surface formed with a helical groove containing a conductor extending between a proximal connector and a distal tip electrode, and an outer insulator composed of polyurethane or silicone rubber or a combination thereof is positioned around the inner insulator. The lead system may be formed with a lumen for receiving a guidewire longitudinally through the lead body. A pair of insulated conductors may be received in the helical groove, one extending to the distal tip electrode, the other extending to a ring electrode proximally spaced from the distal tip electrode.

    摘要: A subcutaneous implantable cardiac device system applies defibrillation currents in pathways aligned with the heart's own electrical system to decrease defibrillation thresholds. In one implementation, one or more subcutaneous sensors detect ventricular fibrillation. Positioning of subcutaneous sensors and filtering result in improved sensing with reduced noise. A subcutaneous patch component of the system that is in communication with a subcutaneous pacemaker or cardioverter-defibrillator may perform the sensing and apply the defibrillation. The subcutaneous patch may include one or more electrodes that perform both sensing and defibrillation. Variations of the subcutaneous patch may include battery and capacitor for generating onboard defibrillation current and may also include a microprocessor for advanced programmable operation.

    摘要: Systems and methods for positioning implanted devices in a patient are disclosed. A method in accordance with a particular embodiment includes, for each of a plurality of patients, receiving a target location from which to deliver a modulation signal to the patient's spinal cord. The method further includes implanting a signal delivery device within a vertebral foramen of each patient, and positioning an electrical contact carried by the signal delivery device to be within ±5 mm. of the target location, without the use of fluoroscopy. The method can still further include, for each of the plurality of patients, activating the electrical contact to modulate neural activity at the spinal cord. In further particular embodiments, RF signals, ultrasound, magnetic fields, and/or other techniques are used to locate the signal delivery device.
