A fiber-optic waveguide, used for ablating lesions in blood vessels, is mounted within and guided by a catheter having multiple lumens extending therethrough and parallel to each other. The waveguide fits within at least one lumen and a guidewire, previously inserted in a blood vessel, extends through another lumen. The distal end of the waveguide can have a short section of larger diameter fiber fused to it to cause a laser beam transmitted through the fiber to expand as it emerges from the waveguide to provide a larger ablation area. The waveguide may also be connected to an energy source by means of an energy coupler.

A laser endoscope includes a micro-thin optic cable assembly which may be inserted into a body cavity by micro-surgical or non-surgical procedures. A coherent jacketed fiber optical bundle is provided in the center of the assembly having a diameter of no more than 3.5 mm. with an optical lens system at the distal end for reflecting an image of a portion of the interior of the cavity. A pair of single optical fibers are spirally wound around the cable, each of which has a total diameter including a jacket of approximately 140 microns. One of these fibers carries a laser light beam to the cavity and the other is for redundancy. The proximate end of the optical cable assembly connects to a remote unit wherein a laser diode light source is provided to direct a beam of light into the end of one of the single optical fibers. A TV camera is also provided in the remote unit together with suitable optics for transmitting the image from the optical cable to a viewing screen on the remote unit.

An optical probe has multiple side-firing optical fibers which terminate in a linearly staggered fashion. A central fiber can be used as well. In diagnostic techniques, one fiber can be used as an emitter, while the others are used as receivers, or various fibers can be used as emitters and receivers at different times to form a map of the area. In therapeutic techniques, the treatment light can be emitted from the fibers in parallel or in sequence, and the fluence can be independently adjusted for each of the fibers. Therapy is readily combined with diagnosis and monitoring by directing the therapeutic light through the central fiber and using the side-firing fibers for reflectance and/or fluorescence spectroscopy before, during, and after therapy.

A deflectable guidewire for a catheter includes a core extending from the proximal end of the guidewire toward the distal end thereof; a hollow sheath for encasing the core; the hollow core including a deflecting region at the distal end of the guidewire, the deflecting region having a first side and a second side, the first side being on an opposite side of a plane extending axially through the hollow means than the second side; the first side of the deflecting region being able to be compressed a greater distance than the second side of the deflecting region; the core being fixed to the hollow sheath at the distal side of the deflecting region; and a control handle mounted at the proximal end of the core and the hollow sheath for applying tension to the core with respect to the hollow sheath so that the tension causes the first side of the deflecting region to compress a greater amount than the second side, thus deflecting the deflecting region. In one embodiment, the core may include laser conducting optical fibers. Other features of the present invention include arranging the optical fibers in the catheter in such a way that bending stresses are distributed evenly throughout the fibers when the catheter is bent. One method of doing this is to arrange the fibers in rows or layers wherein each row or layer is twisted in a direction that is opposite to the direction of the adjacent rows or layers. It has also been found that flexibility is increased without unacceptable transmission losses by using optical fibers having a diameter of about fifty microns.
