摘要:
Methods and devices are provided for, under control of one or more processors within an implantable medical device (IMD), delivering cardiac resynchronization therapy (CRT) at one or more pacing sites. The processors obtain cardiac signals, associated with a candidate beat, from multi-site left ventricular (MSLV) electrodes distributed along a left ventricle and analyze the cardiac signals to collect at least one of a MSLV conduction pattern or a MSLV morphology. The processors compare at least one of the MSLV conduction pattern or MSLV morphology to one or more associated templates. The processors then label the candidate beat as a pseudo-fusion beat based on the comparing and adjust the CRT based on the labeling.

Systems, devices, or methods can be used to detect an event, or series of events, that can indicate worsening of congestive heart failure (CHF), or can be used to identify a subject at an elevated risk for developing CHF. A CHF event predictor can be provided using a characteristic minimum of subject cardiac intervals. In an example, the subject cardiac intervals can be obtained during a night-time period or during periods of reduced subject physical activity. A CHF event predictor can be determined using information about physiologic signals received from a subject, such as from a physiologic sensor associated with an ambulatory or implantable medical device.

A medical device system includes a cardioverter-defibrillator for detecting and treating ventricular tachycardia (VT). The medical device system includes a sensing module for sensing a cardiac signal from available cardiac signal sensing vectors. A control module generates morphology templates of the cardiac signals for multiple patient postures for each of the available sensing vectors and determines a set of posture-independent template features. An unknown cardiac rhythm is classified in response to comparing features of a cardiac signal received during the unknown rhythm to the set of posture-independent features.

This document discusses, among other things, systems and methods to acclimate a patient to therapy from an implantable medical device. For instance, an implantable medical device can include pulse generation circuitry, sensing circuitry, and a controller. The pulse generation circuitry can generate electrical pulses. The sensing circuitry can be for sensing cardiac electrical activity of the patient. In an example, the controller can detect cardiac events that define pacing timing intervals and control the delivery of electrical pulses in accordance with a programmed mode. The controller can be programmed to provide instructions to the pulse generation circuitry to deliver electrical pulses to the heart of a patient. In an example, the electrical pulses can be based on a therapy parameter. The controller can be configured to adjust the therapy parameter according to an acclimation profile to acclimate the patient to a stimulation therapy.

Systems, apparatus, methods and computer-readable storage media that facilitate monitoring the integrity of telemetry connectivity between an implantable device and an external device are provided. In one embodiment, an implantable device includes a monitoring component that monitors advertisement signal information identifying an amount of advertisement signals transmitted to the external device within a defined time period, and telemetry session information identifying an amount of the telemetry sessions that are established between the external device and the implantable device within the defined time period. A connectivity assessment component of the implantable device further determines whether a telemetry connectivity problem exists between the external device and the implantable device based on a degree of miscorrelation between the advertisement signal information and the telemetry session information.

A medical monitoring and treatment device that includes a therapy delivery interface, a plurality of therapy electrodes coupled to the therapy delivery interface, a plurality of electrocardiogram sensing electrodes to sense electrocardiogram signals of a patient, a sensor interface to receive the electrocardiogram signals and digitize the electrocardiogram signals, and at least one processor coupled to the sensor interface and the therapy delivery interface to analyze the digitized electrocardiogram signals, to detect a cardiac arrhythmia based on the digitized electrocardiogram signals, and to control the therapy delivery interface to apply electrical therapy to the patient based upon the detected cardiac arrhythmia. The at least one processor is further configured to analyze a frequency domain transform of the digitized electrocardiogram signals, to determine a metric indicative of a metabolic state of a heart of the patient, and to accelerate or delay application of the electrical therapy based upon the metric.

Systems, methods and implantable devices configured to provide cardiac resynchronization therapy and/or bradycardia pacing therapy. A first device located in the heart of the patient is configured to receive a communication from a second device and deliver a pacing therapy in response to or in accordance with the received communication. A second device located elsewhere is configured to determine an atrial event has occurred and communicate to the first device to trigger the pacing therapy. The second device may be configured for sensing the atrial event by the use of vector selection and atrial event windowing, among other enhancements. Exception cases are discussed and handled as well.

The disclosure herein relates to systems and methods for wireless power transfer between a wireless power outlet and a wireless power receiver. In particular, the disclosure relate to a wireless power receiver enabled to monitor a sensor arrangement associated with the secondary coil of the power receiver and further determine its orientation relative to the wireless power outlet and calculate the electric power loss for an orientation. Furthermore, a monitoring unit associated with the wireless power receiver is operable to receive power transfer and orientation data from the sensor arrangement such as voltage, current, capacity, resistance, temperature and the like, to calculate efficiency data from the sensor data and further operable to communicate the efficiency data to a communication and control.
