    Abstract: A drainage catheter is adapted to drain fluid from the body cavity through a body conduit and includes an elongate tube and a retention member disposed at the distal end of the elongate tube. The retention member is adapted for movement between a low-profile state facilitating insertion of the catheter and a high-profile state facilitating retention of the catheter in its operative position. A woven mesh forms at least a portion of one of the tube and the retention member. The woven mesh can be formed of filaments heat-settable so that the catheter automatically moves to the high-profile state. Insertion of the catheter can be facilitated using a stylet which can be removed after insertion to permit the catheter to automatically return to its high-profile state.

    Abstract: A surgical access device, such as a trocar, includes a pendent valve having an elongate structure extending from a proximal end to a septum valve disposed at a distal end. In operation, the elongate structure follows the angle of the instrument to pre-position the septum valve into the path of the instrument where it is not significantly challenged during instrument insertion or manipulation. The pendant valve can be made to float at both the proximal end and the distal end of the elongate structure, to further reduce the vulnerability of the septum valve.     Abstract: A surgical staple-clip including a clip component and a securing member is used in a wide range of surgical procedures. The staple-clip may be introduced to a surgical site in an un-assembled condition through a small port or trocar. An applier for the staple-clip comprising a pair of opposed jaw-like channels is provided to position and apply the clip component and the securing member. The clip component is positioned around a target tissue and is compressed or clamped upon the tissue using only the force required for a specific surgical procedure such as occlusion, ligation or fixation. When the clip component is properly applied, the securing member is urged forward and over the clip component to secure the staple-clip. The clip component may include traction enhancement features such as surface interruptions, bumps, valleys and ridges. With the staple-clip of the invention, the force required to constrict or occlude the tissue is separate from the force required to secure and maintain the staple-clip in position and, as a result, the body tissue is not over-compressed and nourishment to the body tissue is maintained. Other aspects of the invention include thumb actuated clip appliers for use in hand assisted laparoscopy (HAL). In one embodiment, a clip applier includes a handle and a thumb actuated mechanism that is used to slidably release clips onto a body tissue or vessel by sliding the thumb actuated mechanism forward and backward using only one hand. In another aspect of the invention, a two-stage clip is disclosed having a clip component and a staple component for securing the clip after it has been properly positioned.

    Abstract: An improved clip or clamp is provided having a one-piece wire-form construction that is suited for rapid and repeatable manufacture. The clamp, which may be disposable or reusable, includes a first jaw portion, a second jaw portion, a first handle portion, a second handle portion and a central coil or tensioning device, all of which are formed from a single piece of wire. The central coil or tensioning device comprises one or more turns. The handle portions may include ring-shaped distal ends that further enhance grip. The jaw portions may be shaped, curved, bent or otherwise configured to provide access to specific areas of a procedure. The clamp may be coated with a material providing improved traction and padding. In a method of the invention, a single wire is wound at least one turn forming the central coil and two extensions. A first extension of the wire is formed into the first jaw portion by bending back on itself, the first extension is then returned to the central coil where it is formed to rest upon an outer surface of the central coil. The bent first extension, which now extends rearward in a direction opposite to the direction of the first jaw portion, is then bent again forming the first handle portion. A second extension is similarly bent to form the second jaw portion and the second handle portion. The single-wire construction of the clamp may be achieved through programmed wire forming. In another aspect of the invention, a plurality of clamps may be placed upon a traction rod made from a metal or plastic material to align two or more stump portions of a severed vessel or tissue for reconnection or anastomosis.
